摘要
目的探讨胃肠超声助显剂在胃食管反流病及胃食管结合部病变筛查中的诊断价值。方法选取2016年6月至2019年6月青岛市立医院西院区收治的胃食管反流病患者100例为A组,同期100例健康志愿者为B组。两组均行口服胃肠超声造影胃肠超声助显剂,测量并比较His角度和腹段食管,比较5 min内反流次数和反流时间,以及超声造影、胃镜检查及病理诊断胃食管结合部病变结果。结果A组的His角大于B组[(83.0±9.1)°比(73.2±9.3)°],A组的腹段食管长度短于B组[(2.35±0.24)cm比(2.50±0.21)cm],差异有统计学意义(P<0.05)。A组反流次数≥3次者89例,反流时间≥3 s者99例;B组反流次数≤2次者100例,反流时间≤2 s者98例,差异有统计学意义(P<0.05)。超声造影结果显示诊断准确率为85.19%,与胃镜的88.89%比较,差异无统计学意义(P>0.05)。结论随着His角度的增大及腹段食管的缩短,胃食管反流率升高,临床中可通过口服胃肠超声助显剂进行诊断。
Objective To explore the diagnostic value of gastrointestinal ultrasound assistant in gastroesophageal reflux and gastroesophageal junction lesion screening.Methods One hundrd patients with gastroesophageal reflux from June2016 to June 2019 were taken as group A,and 100 healthy volunteers were taken as group B during the same period.All the subjects in the two groups underwent oral gastric window ultrasound contrast,and the gastrointestinal ultrasound assistant was taken orally five minutes before the examination,and the images were frozen after the images were clear.The His angle and abdominal esophagus were measured and compared,the number of reflux times and the reflux time within five minutes were compared,and the results of ultrasound contrast with gastroscopy and pathological diagnosis of gastroesophageal junction were compared.Results The His angle in group A was significantly larger than that in group B[(83.0±9.1)°vs.(73.2±9.3)°,P<0.05].The length of abdominal esophagus in group A was significantly shorter than that in group B[(2.35±0.24)cm vs.(2.50±0.21)cm,P<0.05].In group A,89 cases had reflux times≥three times,and 99 cases had reflux time≥three seconds.In group B,100 cases had reflux times≤two times,and 98 cases had reflux time≤two seconds,the difference was statistically significant(P<0.05).The diagnostic accuracy rate of ultrasound contrast was 85.19%which had no significant difference of gastroscope(88.89%,P>0.05).Conclusions With the increase of the His angle and the shortening of the abdominal esophagus,the gastroesophageal reflux rate increases.In the clinic,it can be diagnosed by oral gastrointestinal ultrasound.
