新生儿化脓性脑膜炎患儿脑脊液IL-6,IL-10的临床价值

Value of detecting cerebrospinal fluid levels of IL-6 and IL-10 in newborn with purulent meningitis

目的探讨新生儿化脓性脑膜炎脑脊液中白介素-6(IL-6)及白介素-10(IL-10)的临床价值。方法选取2017年1月~2018年12月入住昆明市儿童医院新生儿科96例新生儿,根据《实用新生儿学》第四版中对化脓性脑膜炎的相关诊断标准进行判定,将确诊的新生儿化脓性脑膜炎的43例新生儿归为观察组,非化脓性脑膜炎新生儿53例归为对照组,对两组新生儿脑脊液中IL-6,IL-10水平进行检测,并对两组患儿脑脊液中IL-6,IL-10水平及其和白细胞数的相关性进行分析。结果观察组(化脑组)脑脊液中IL-6及IL-10含量明显高于对照组,特别是IL-6水平明显升高,对比两组间差异均有统计学意义(P<0.01);两组患儿脑脊液中白细胞与IL-6,IL-10间未见显著相关性(P>0.05)。结论IL-6,IL-10在化脓性脑膜炎的病理生理过程中发挥重要作用。新生儿化脓性脑膜炎诊断中检测脑脊液中IL-6,IL-10水平作为一种重要指标,特别是IL-6对早期诊断新生儿化脑的价值显著,可使漏诊率有效降低。

Objective To study the clinical value of cerebrospinal fluid(CSF)IL-6 and IL-10 in newborn with purulent meningitis(PM).Methods From January 2017 to December 2018,96 newborns admitted to the department of neonatology of Kunming children's hospital were selected.According to the relevant diagnostic criteria for purulent meningitis in the fourth edition of practical neonatology,43 newborns with confirmed purulent meningitis were classified into the observation group and 53 newborns with non-purulent meningitis were classified into the control group.The levels of il-6 and il-10 in cerebrospinal fluid of the two groups of newborns were detected,and the correlation between the levels of il-6 and il-10 in cerebrospinal fluid of the two groups of children and the number of leukocytes was analyzed.Results The levels of IL-6 and IL-10 in purulentmeningitis groupwere significantly higher than those in control group(<0.01),and there were no correlation between the CSF level of IL-6,IL-10 and leucocyte in these two groups(>0.05).Conclusions IL-6 and IL-10 play an important role in the pathophysiological process of purulent meningitis.Detection of IL-6 and IL-10 levels in cerebrospinal fluid is an important indicator in the diagnosis of neonatal purulent meningitis,especially IL-6 has significant value in early diagnosis of neonatal encephalomyelitis,which can effectively reduce the missed diagnosis rate.
