基于移动管理系统的延续护理联合中高架势八段锦对溃疡性结肠炎患者的负性情绪、生活质量和自我效能感的影响

Effect of continuous nursing based on mobile management system combined with middle-upper posture Baduanjin on negative emotion,quality of life,and self-efficacy of patients with ulcerative colitis

目的探讨基于移动管理系统的延续护理联合中高架势八段锦对溃疡性结肠炎患者的负性情绪、生活质量和自我效能感的影响。方法将2018年5月至2019年10月间山东省立第三医院消化内科收治的96例溃疡性结肠炎患者按照随机数表法分为对照组和观察组,每组48例。对照组采取常规护理措施,观察组在此基础上利用移动管理系统实施延续护理与中高架势八段锦干预。比较两组患者出院时、出院3个月的抑郁自评量表(SDS)、焦虑自评量表(SAS)、自我效能感量表(GSES)与生活质量综合评定问卷(GQOLI-74)评分的变化。结果出院时,两组患者的SAS评分和SDS评分比较差异均无统计学意义(P>0.05);出院3个月,观察组和对照组患者的SAS评分[(34.22±2.83)分vs(39.87±2.55)分]和SDS评分[(34.06±4.06)分vs(39.93±5.35)分]比较,观察组明显低于对照组,差异均有统计学意义(P<0.05);出院时,两组患者的自我护理责任感评分、自我护理能力评分、自我决策评分、自我减压评分比较差异均无统计学意义(P>0.05);出院3个月,观察组和对照组患者的自我护理责任感评分[(34.63±2.85)分vs(28.52±2.87)分]、自我护理能力评分[(30.35±3.63)分vs(28.64±3.81)分]、自我决策评分[(30.65±2.52)分vs(28.34±2.86)分]、自我减压评分[(25.56±3.55)分vs(23.45±4.24)分]比较,观察组明显高于对照组,差异均有统计学意义(P<0.05);出院时,两组患者的社会功能评分、心理功能评分、躯体功能评分与生活状态评分比较差异均无统计学意义(P>0.05);出院3个月,观察组和对照组患者的社会功能评分[(60.56±5.34)分vs(56.61±4.83)分]、心理功能评分[(64.68±4.52)分vs(57.66±3.89)分]、躯体功能评分[(60.55±6.33)分vs(54.34±5.02)分]与生活状态评分[(54.66±5.23)分vs(48.62±4.06)分]比较,观察组明显高于对照组,差异均有统计学意义(P<0.05)。结论基于移动管理系统的延续护理联合中高架势八段锦能够纠正溃疡性结肠炎患者的负性情绪,增进其自我效能感,提高其生活质量,值得临床推广应用。

Objective To explore the effect of continuous nursing based on mobile management system combined with middle-upper posture Baduanjin on the negative emotion,quality of life,and self-efficacy of patients with ulcerative colitis.Methods Ninety-six patients of ulcerative colitis who were treated in Department of Gastroenterology,Shandong Provincial Third Hospital from May 2018 to October 2019 were selected and randomly divided into control group and study group,with 48 patients in each group.The control group took routine nursing measures,and the study group used the mobile management system to implement continuous nursing and the middle-upper posture Baduanjin intervention.The scores of Self-rating Depression Scale(SDS),Self-rating Anxiety Scale(SAS),Self-efficacy Scale(GSEs),and Comprehensive Quality of Life Questionnaire(GQOLI-74)were compared between the two groups at discharge and 3 months after discharge.Results At discharge,there were no statistically significant differences in SAS score and SDS score between the two groups(P>0.05);three months after discharge,the SAS score and SDS score of patients in the observation group were significantly lower than those in the control group(P<0.05):SAS score,(34.22±2.83)points vs(39.87±2.55)points;SDS score,(34.06±4.06)points vs(39.93±5.35)points.At discharge,there were no statistically significant differences between the two groups in self-care responsibility score,self-care ability score,self-decision score,and self-decompression score(P>0.05);three months after discharge,the observation group was significantly higher that the control group in the scores(P<0.05):self-care responsibility score,(34.63±2.85)points vs(28.52±2.87)points;self-care ability score,(30.35±3.63)points vs(28.64±3.81)points;self-decision score,(30.65±2.52)points vs(28.34±2.86)points;self-decompression score,(25.56±3.55)points vs(23.45±4.24)points.At discharge,there were no statistically significant differences in social function score,psychological function score,physical function score,and life status score between the two groups(P>0.05);three months after discharge,the observation group was significantly higher than the control group in the above indexes(P<0.05):social function score,(60.56±5.34)points vs(56.61±4.83)points;psychological function score,(64.68±4.52)points vs(57.66±3.89)points;physical function score,(60.55±6.33)points vs(54.34±5.02)points;and life status score,(54.66±5.23)points vs(48.62±4.06)points.Conclusion The continuous nursing based on the mobile management system combined with middle-upper posture Baduanjin can correct the negative emotions of patients with ulcerative colitis,enhance their self-efficacy and improve their quality of life,which is worthy of clinical promotion.
