脑小血管病患者相关指标的变化研究

Study on changes of related indexes in patients with cerebral small vessel disease

目的观察脑小血管病(CSVD)患者同型半胱氨酸(Hcy)、尿酸(UA)及脉动指数(PI)的变化。方法选取CSVD患者212例,其中脑白质病变(WML)组116例,腔隙性梗死(LI)组96例,同期就诊的非脑小血管病患者80例为对照组。采用血液检测及颅脑MRI、TCD检查,测定血Hcy、UA水平与PI数值。结果WML组及LI组Hcy、UA、PI水平与对照组比较,差异有统计学意义(P<0.01);WML组Hcy、UA、PI显著高于LI组(P<0.05);LI组Hcy与UA呈显著相关。结论血Hcy、UA升高水平与脑白质损害程度关联,血浆Hcy、UA及PI可作为脑白质损害严重程度的一项指标。

Objective To observe the changes of homocysteine(Hcy),uric acid(UA)and pulsation index(PI)in patients with cerebral small vessel disease(CSVD).Methods A total of 212 patients with CSVD were selected and di⁃vided into two subgroups:116 cases in white matter lesions(WML)group and 96 cases in lacunar infarction(LI)group.80 cases of non-cerebrovascular disease patients during the same period were in the control group.Blood tests,brain MRI and TCD examinations were used to determine blood Hcy,UA levels and PI values.Results The Hcy,UA,PI lev⁃els of WML group and LI group were compared with control group,the difference was statistically significant(P<0.01);Hcy,UA,PI of WML group were significantly higher than those in LI group(P<0.05);the levels of Hcy and UA in LI group were significantly related.Conclusion The elevated level of blood Hcy and UA are associated with the de⁃gree of white matter damage.Plasma Hcy,UA and PI can be used as an indicator of the severity of white matter damage.
