布地奈德雾化吸入治疗婴幼儿吸入性肺炎的效果及对IL-6、IL-8、TNF-α的影响 被引量:10

摘要 目的观察布地奈德混悬液雾化吸入治疗婴幼儿吸入性肺炎的临床疗效及对患儿血清白介素-6(IL-6)、IL-8和肿瘤坏死因子α(TNF-α)的影响。方法选择呼吸科于2017年6月至2019年6月收治的80例吸入性肺炎患儿作为研究对象,根据治疗方案不同分为对照组和观察组,每组40例。2组患儿均接受常规治疗,观察组另外给予雾化吸入布地奈德混悬液。比较2组主要临床症状消失时间及治疗前后血气指标[二氧化碳分压(PaCO2)和氧分压(PaO2)]和炎性指标(IL-6、IL-8和TNF-α)等变化情况。结果观察组患者发绀、呼吸困难和湿啰音等消失时间显著少于对照组,差异有统计学意义(P<0.05)。观察组患儿治疗后PaO2水平显著高于对照组,PaCO2水平、血清IL-6、IL-8和TNF-α水平显著低于对照组,差异有统计学意义(P<0.05)。结论在常规治疗基础上,雾化吸入布地奈德治疗婴幼儿吸入性肺炎安全、有效,能有效改善临床症状,缓解炎症状态。 Objective To investigate the therapeutic effects of budesonide atomization inhalation in treatment of infantile aspiration pneumonia and its effects on the serum levels of interleukin-6(IL-6),IL-8 and tumor necrosis factorα(TNF-α).Methods Eighty infants with aspiration pneumonia who were treated in our hospital from June 2017 to June 2019 were enrolled in the study,who were divided into control group and observation group according to different treatment schemes,with 40 cases in each group.The patients in control group were treated by routine therapies,however,the patients in observation group,on the basis of control group,were treated by budesonide suspension aerosol inhalation.The curative effects,and the disappearance time of main clinical symptoms,the PaCO2,PaO2,IL-6,IL-8 and TNF-αwere observed and compared between the two groups.Results The disappearance time of cyanosis,dyspnea and wet rales in observation group was significantly less than that in control group(P<0.05).After treatment,the levels of PaO2 in observation group were significantly higher than those in control group,however,the levels of PaCO2,IL-6,IL-8,TNF-αin observation group were significantly lower than those in control group(P<0.05).Conclusion Budesonide in treatment of infantile aspiration pneumonia is safe and effective,which can effectively improve clinical symptoms and relieve inflammation status of patients.
