摘要
目的探讨妊娠中期孕妇血清铁蛋白(SF)水平与甲状腺功能的相关性。方法纳入2016年12月~2017年7月在我院产科门诊常规产检的妊娠中期孕妇1484例,根据是否存在ID(SF<20μg/L)将其分为铁缺乏症(ID)组348例和正常组1136例。收集并比较两组的一般资料、常规实验室指标及甲状腺功能和甲状腺自身抗体,分析妊娠中期孕妇SF水平与甲状腺功能的相关性。结果与正常组比较,ID组孕妇的促甲状腺激素(TSH)水平明显升高,游离甲状腺素(FT 4)水平明显降低(P<0.05),而两组间甲状腺过氧化物酶抗体(TPOAb)和甲状腺球蛋白抗体(TgAb)比较差异无统计学意义(P>0.05)。相关性分析结果显示,妊娠中期孕妇SF与TSH水平呈负相关(r=-0.120,P<0.001),与FT 4呈正相关(r=0.205,P<0.001)。logistic回归分析结果显示,SF水平降低是TSH水平升高和FT 4水平降低的危险因素(β=-0.002,P=0.022;β=0.005,P=0.003)。结论SF水平降低是妊娠中期孕妇出现甲状腺功能异常的危险因素。
Objective To explore the correlation between serum ferritin(SF)level and thyroid function in the second trimester of pregnancy.Methods A total of 1484 women in the second trimester who received routine check-ups in our hospital’s obstetrics clinic from December 2016 to July 2017 were included.They were divided into the iron deficiency(ID)group(348 cases)and the normal group(1136 cases)according to whether existed ID(SF<20μg/L).General data,routine laboratory indicators,thyroid function and thyroid autoantibodies of the two groups were collected and compared,and the correlation between SF levels and thyroid function in pregnant women in the second trimester was analyzed.Results Compared with the normal group,thyroid stimulating hormone(TSH)level was significantly higher and the free thyroxine(FT 4)level was significantly decreased in the ID group(P<0.05).There were no significant differences in thyroid peroxidase antibody(TPOAb)and thyroid globulin antibody(TgAb)between the two groups(P>0.05).Correlation analysis showed that SF was negatively correlated with TSH in second trimester women(r=-0.120,P<0.001),while positively correlated with FT 4(r=0.205,P<0.001).Logistic regression analysis showed that SF level reduction was a risk factor for TSH level increase and FT 4 level decrease(β=-0.002,P=0.022;β=0.005,P=0.003).Conclusion Reduced SF level is a risk factor for thyroid dysfunction in pregnant women in the second trimester.
