基层糖尿病甲病护理队伍培养模式的构建与实践探索

The construction and practice exploration of cultivating model of community nursing team of diabetic nail abnormalities

目的构建基层糖尿病甲病护理队伍培养模式。方法医院足病专科护理团队为基层专科护士建立糖尿病甲病护理队伍培养方案,从理论模块和技能模块2个方面进行,通过讲座、视频、工作坊、示教和实训的形式,对社区护士进行糖尿病甲病专科护士培养。结果培养模式实施3年来,共培养了58名社区糖尿病专科护士,7家社区卫生服务中心建立了糖尿病甲病门诊,平均每个门诊糖尿病患者畸形趾甲就诊20人次/月,总计处理畸形趾甲890人次,健康教育1200人次,嵌甲转归正常者由4例增加为33例,畸形甲引起溃疡者由12例降为2例。结论医院与社区联动的糖尿病甲病护理队伍培养模式可为基层专科护士提供专业化的糖尿病甲病护理教育资源平台,提高其专科知识与技能水平,对社区糖尿病甲病的防治产生积极的作用。

Objective To construct the cultivating model of community nursing team of diabetic nail abnormalities.Methods The specialized nursing team of diabetic foot in the hospital established a training program for the nursing team of diabetic foot in the community,which was carried out from two aspects of theory module and skill module.The community nurses were trained in the form of lectures,videos,workshops,teaching and practical training.Result In the past three years,a total of 58 community diabetes specialist nurses have been trained,and 7 community health service centers have established community nursing clinics.Among them,5 clinics were established for 1 year,and 2 clinics were established for 2 years.The average number of patients with deformed toenails in each outpatient department was 20 person times/month.The total number of patients with abnormal toenails was 890 person-time,health education 1200 person-time.The number of patients with normal ingrown toenails was increased from 4 cases to 33 cases,and the ulcer caused by malformed nail decreased from 12 cases to 2 cases,which avoided the occurrence and development of foot ulcer.Conclusion The cultivating mode of nursing team of diabetic nail abnormalities linked by hospital and community can provide professional nursing education resource platform for primary specialized nurses,improve their professional knowledge and skills,play a positive role in the prevention and treatment of diabetic nail abnormalities in the community,and effectively prevent the occurrence of diabetic foot.
