眼轮匝肌节制韧带眶周定点方法在下睑肿瘤切除后修复重建中的应用

Application of periorbital anchor point suture suspension of orbicularis retaining ligament in the repair and reconstruction of lower eyelid after tumor resection

目的探讨眼轮匝肌节制韧带(orbicularis retaining ligament, ORL)眶周定位方法在下睑肿瘤切除后修复重建中的应用效果。方法对22例因下睑肿瘤切除后致下睑皮肤缺损的患者,根据ORL在眶周分布特点,采用眶缘ORL松解加悬吊下睑眶周定点方法治疗。结果本组22例患者术后下睑力学稳定性得到了很好的恢复,外观自然;术后随访3~6个月,未见外翻和退缩的情况。结论 ORL的眶周定点用于下睑肿瘤切除后修复重建,其瘢痕较隐蔽、下睑力学稳定性较持久,值得在临床应用与推广。

Objective To explore the efficacy of periorbital anchor point suture suspension of orbicularis retaining ligament(ORL) in the repair and reconstruction of lower eyelid after tumor resection. Methods For 22 patients with lower eyelid skin defect after tumor resection, orbital margin ORL release and lower eyelid periorbital anchor point suture suspension was performed according to the distribution of ORL. Results The mechanical stability of lower eyelid was recovered well in 22 patients and the appearance was natural.No ectropion and retraction was found during the follow-up from 3 to 6 months. Conclusion The application of periorbital anchor point suture suspension of ORL in the repair and reconstruction of lower eyelid after tumor resection has advantages of concealed scar and long-lasting mechanical stability of lower eyelid. It should be considered for clinical application and popularization.
