摘要
目的探讨CT模拟机及模拟定位机在颅内肿瘤调强放射治疗(IMRT)靶区勾画与计划制定中的应用。方法选择2018年1月至2018年12月行IMRT的110例颅内肿瘤患者为研究对象,采用随机数字表法,将110例患者分为CT组和X线组,各55例,观察组行CT模拟机IMRT,对照组行模拟定位机IMRT;在CT图像的X线图像上分别进行靶区勾画和IMRT计划制定。结果CT组大体肿瘤体积(GTV)、临床靶区体积(CTV)均高于X线组(P<0.05);两组Y轴方向上的等级误差有显著差异(P<0.05),X轴和Z轴方向上的等级误差无显著差异(P>0.05);X线组Y轴方向上平均误差大于CT组(P<0.05),两组X轴和Z轴方向上平均误差无显著差异(P>0.05);在制定治疗计划中,两组GTV一般会外放2mm。结论CT模拟机及X线模拟定位机都适用于颅内肿瘤IMRT靶区勾画与计划制定,CT模拟机更有助于提高靶区勾画准确性,为IMRT计划制定提供可靠依据。
Objective To explore the application of CT simulator and simulated positioner in the target volume delineation and planning formulation of intensity-modulated radiotherapy(IMRT)of intracranial tumors.Methods A total of 110 patients with intracranial tumors who underwent IMRT from January 2018 to December 2018 were selected for the study,and 110 patients were divided into CT group and X-ray group according to the random number table method,with 55 cases in each group.Observation group was given CT simulator IMRT,and control group was given simulated positioner IMRT.The target volume delineation and IMRT planning formulation were performed on the CT images and X-ray images.Results The gross tumor volume(GTV)and clinical target volume(CTV)in CT group were higher than those in X-ray group(P<0.05).There were significant differences in the grade errors in the Y-axis direction between the two groups(P<0.05),and there were no significant differences in the grade errors in X-axis and Z-axis directions(P>0.05).The average error in Y-axis direction in X-ray group was larger than that in CT group(P<0.05),and there was no significant difference in the average error in X-axis and Z-axis directions(P>0.05).In the formulation of treatment plan,the GTV in the two groups would generally put 2mm outside.Conclusion CT simulator and X-ray simulated positioner are suitable for the IMRT target volume delineation and planning formulation of intracranial tumors.CT simulator is more helpful to improve the accuracy of target volume delineation and provide a reliable basis for IMRT planning formulation.
