颈动脉彩超联合头颅MRI在脑梗死病情评估中的价值研究

Value of Carotid Color Ultrasonography Combined with Cranial MRI in the Disease Evaluation of Cerebral Infarction

目的探讨颈动脉彩超联合头颅MRI在脑梗死病情评估中的临床应用价值。方法选取2017年1月至2017年12月于我院神经内科诊治的60例脑梗死患者作为观察对象,所有患者均进行颈动脉彩超联合头颅MRI检查的同时,给予相应临床治疗,并随访1年。分析临床病情严重程度、MRI分度和远期预后与颈动脉超声改变的相关性。结果随着患者颈动脉狭窄的严重,不稳定性斑块检出率明显增加,呈现上升趋势;随着患者病情的加重,颈动脉狭窄程度亦呈加重趋势;随着MRI检查结果的不断严重,颈动脉狭窄程度明显加重;颈动脉狭窄程度与患者临床病情程度、斑块检出率及MRI分度呈显著正相关。结论颈动脉彩超能提供颈动脉斑块负荷和颈部动脉狭窄情况,为颅内动脉负荷的评估提供有效辅助依据,MRI成像能直观显示病变部位、范围及继发的脑梗死灶范围、位置,评估灌注参数有助于反映患者病情程度。

Objective To evaluate the clinical application value of carotid color ultrasonography combined with cranial MRI in the disease evaluation of cerebral infarction.Methods A total of 60 cases of patients with cerebral infarction in the neurology department of our hospital from January 2017 to December 2017 year were selected as the subjects.All patients were treated with carotid color ultrasonography combined with cranial MRI,and given corresponding clinical treatment,and they were followed up for 1 year.The correlation between clinical disease severity,MRI graduation,longterm prognosis and carotid ultrasound changes was analyzed.Results With the severity of carotid artery stenosis,the detection rate of unstable plaque was increased obviously and showed an upward trend.With the aggravation of the patients’condition,the carotid stenosis degree was also aggravated.With the continuous severity of MRI examination,the carotid stenosis degree was obviously aggravated.The carotid stenosis degree was significantly positively correlated with the clinical disease severity,plaque detection rate and MRI graduation.Conclusions Color Doppler ultrasound of carotid artery can provide carotid plaque location and carotid artery stenosis,and provide effective auxiliary basis for the evaluation of intracranial artery load.MRI imaging can directly display the location and scope of lesions and the scope and location of secondary cerebral infarction.Evaluation of perfusion parameters can help to reflect the patient's condition.
