本体感觉和平衡训练对全膝关节置换术后功能恢复的影响

摘要 目的观察本体感觉和平衡功能治疗全膝关节置换术后的疗效。方法选取2017年1月~2018年6月在我院骨科行初次单侧全膝关节置换术后(TKA)患者50例,采用随机数字表法分为观察组和对照组,每组25例。两组患者均给予常规康复治疗,观察组在常规康复治疗基础上给予本体感觉和平衡功能训练,并于康复治疗前和康复治疗后分别采用静态平衡仪检测平衡能力,Berg平衡量表和膝关节评分量表进行评定膝关节恢复情况。结果两组患者康复治疗前的膝关节功能各项评分比较,差异无统计学意义(P>0.05)。两组患者康复治疗后的BBS评分和HSS评分比较,观察组均较对照组明显改善,差异具有统计学意义(均P<0.05)。观察组患者康复治疗后ΔX(ec-eo)、ΔY(ec-eo)、ΔA^2(ec-eo)、ΔD(ec-eo)、ΔV(ec-eo)评分均较对照组明显改善,两组比较差异具有统计学意义(均P<0.05)。结论本体感觉和平衡功能训练结合常规康复治疗能明显改善全膝关节置换术后平衡功能和膝关节功能,有效促进膝关节功能恢复,提高患者生活质量。 Objective To observation the clinical efficacy of total knee arthroplasty by proprioception training,balance training and routine rehabilitation training.Methods 50 patients who underwent primary unilateral total knee arthroplasty(TKA)in our hospital from January 2017 to June 2018 in our hospital were selected,they were divided into observation group(25 cases)and control group(25 cases)according to the random number table mrthod.The control group was treated with routine rehabilitation taining.The observation group was given proprioception and balance function training on the basis of routine rehabilitation treatment.The Knee joint balance function of static balanc apparatus,Berg Balance Scale(BBS)and the Hospital for Special Surgery(HSS)score were measured and compared in the two groups on rehabilitation treatment beginning,4 weeks after the rehabilitation treatment.Results The difference was not statistically significant(P>0.05)in balance function,BBS and HSS scores between the two groups before rehabilitation treatment.The ΔX(ec-eo),ΔY(ec-eo),ΔA^ 2(ec-eo),ΔD(ec-eo)andΔV(ec-eo)scores were(9.28±3.12)mm,(13.64±2.78)mm,(354.80±126.92)mm 2,(303.68±120.15)mm and(4.34±2.14)mm in observation group after the rehabilitation treatment.The control group were(11.88±3.40)mm,(21.32±3.26)mm,(431.60±136.74)mm 2,(379.16±129.31)mm and(6.20±2.08)mm.The difference had statistical significance in both two groups(P<0.05).The BBS,HSS scores were(40.52±6.06)scores and(84.20±5.21)scores in observation group after treatment.The control group were(32.36±5.62)scores and(78.96±5.37)scores,The difference had statistical significance in both two groups(P<0.05).Conclusion The proprioception training,balance training and routine rehabilitation training after the total knee replacement can accelerate the recovery of knee joint function include balance function and improve the quality of life.
