摘要
目的观察冠脉使用非离子型碘造影剂对血钾水平的影响,明确低钾血症的发生与造影剂剂量的关系。方法回顾性分析河南中医药大学第一附属医院和嵩县中医院行冠状动脉造影或介入治疗的148例患者的临床资料,其中造影剂使用量为30~100 mL(低剂量组)72例,>100 mL(高剂量组)76例。分析手术前、后血钾水平的变化及不同造影剂剂量(高剂量组、低剂量组)和性别对低钾血症发生率的影响。结果低剂量组、高剂量组手术后血钾水平均显著低于手术前(P<0.05)。高剂量组低钾血症发生率高于低剂量组,女性低钾血症发生率高于男性,但差异均无统计学意义(P>0.05)。结论冠脉内使用非离子型碘造影剂可导致临床低钾血症的发生,但与造影剂剂量无明显相关性。
Objective To observe the effect of using non-ionic iodine contrast agent in coronary artery on blood potassium level,and to clarify the relationship between hypokalemia and the dose of contrast agent.Methods The clinical data of 148 patients who underwent coronary angiography or interventional therapy in the first affiliated hospital of Henan university of CM and traditional Chinese medicine hospital of Song county were analyzed retrospectively.Among them,72 cases used 30-100 mL contrast agent(low-dose group),and 76 cases used >100 mL contrast agent(high-dose group).The changes of blood potassium level before and after operation and the effects of different contrast agent doses(high-dose group,low-dose group)and gender on the incidence of hypokalemia were analyzed.Results The serum potassium level in the low-dose group and the high-dose group after operation were significantly lower than those before operation(P<0.05).The incidence of hypokalemia in the high-dose group was higher than that in the low-dose group,the incidence of hypokalemia in female was higher than that in male,but the differences were not statistically significant(P>0.05).Conclusion The use of non-ionic iodine contrast agent in coronary artery can lead to the occurrence of clinical hypokalemia,but there is no significant correlation between the dosage of contrast agent and hypokalemia.
