摘要
目的分析儿童难治性肺炎支原体(MP)大叶性肺炎的临床特征。方法选取我院收治的95例MP感染导致的大叶性肺炎患儿作为研究对象,依据是否为难治性MP肺炎(RMPP)将患儿分为难治组(48例)和普通组(47例)。比较两组患儿的临床资料及实验室检查结果。结果难治组的发热热程、院前症状持续时间均明显长于普通组,肺部听诊出现呼吸音减低的患儿占比明显高于普通组(P<0.05)。难治组的C反应蛋白、乳酸脱氢酶及D-二聚体水平明显高于普通组(P<0.05)。难治组出现两个及以上肺叶受累的患儿占比明显高于普通组(P<0.05)。结论MP引起的大叶性肺炎难治组患儿的发热热程、院前症状持续时间长,出现呼吸音减低的比例高,外周血中的C反应蛋白、乳酸脱氢酶及D-二聚体的水平高,肺部影像学改变较严重。
Objective To analyze the clinical characteristics of refractory mycoplasma pneumoniae(MP)lobar pneumonia in children.Methods A total of 95 children with lobar pneumonia caused by MP infection admitted in our hospital were selected as the research objects.According to whether the children were refractory mycoplasma pneumoniae pneumonia(RMPP),the children were divided into refractory group(48 cases)and normal group(47 cases).The clinical data and laboratory test results of the two groups were compared.Results The duration of fever and prehospital symptoms in the refractory group were significantly longer than those in the normal group,and the proportion of children with decreased respiratory sounds during auscultation was significantly higher than that in the normal group(P<0.05).The levels of C-reactive protein,lactate dehydrogenase and D-dimer in the refractory group were significantly higher than those in the normal group(P<0.05).The proportion of children with two or more lobes involved in the refractory group was significantly higher than that in the normal group(P<0.05).Conclusion Children of lobar pneumonia caused by MP in the refractory group have a long fever course,long duration of prehospital symptoms,a high proportion of reduced breath sounds,high levels of C-reactive protein,lactate dehydrogenase and D-dimer in peripheral blood,and the serious lung imaging changes.
