腹腔镜与开腹手术治疗子宫肌瘤合并糖尿病的效果和安全性对比观察

Comparative Observation on the Efficacy and Safety of Laparoscopy and Laparotomy in the Treatment of Uterine Fibroids with Diabetes

目的探究对子宫肌瘤合并糖尿病患者实施腹腔镜手术、开腹手术治疗的效果。方法随机将2016年1月—2018年9月该院60例子宫肌瘤合并糖尿病患者分为对照组(30例,应用开腹手术治疗)、实验组(30例,应用腹腔镜手术治疗)。随访1年,对比两组肌瘤残留率、肌瘤复发率、术后首次排气时间、血糖指标、住院时间及并发症发生率。结果实验组术后首次排气时间及住院时间均较对照组更少(P<0.05);实验组术后1 d后空腹血糖及术后1 d餐后2 h血糖较对照组更低(P<0.05);实验组并发症发生率3.33%低于对照组23.33%(P<0.05);实验组肌瘤残留率、肌瘤复发率和对照组差异无统计学意义(P>0.05)。结论对子宫肌瘤合并糖尿病患者实施腹腔镜手术治疗的效果和开腹手术相当,但腹腔镜手术的创伤更小,患者术后恢复更快,且安全性更高。

Objective To explore the effect of laparoscopic surgery and open surgery on patients with uterine fibroids and diabetes.Methods Sixty patients with uterine fibroids and diabetes mellitus from January 2016 to September 2018 were randomly divided into a control group(30 cases with open surgery)and an experimental group(30 cases with laparoscopic surgery).Followed up for 1 year,compared the residual rate of fibroids,the rate of recurrence of fibroids,the time of first postoperative exhaustion,blood glucose index,hospitalization time and the incidence of complications.Results The time of first exhaustion and hospitalization in the experimental group were less than those in the control group(P<0.05);the fasting blood glucose 1 d after the operation and the blood glucose 2 h after the meal 1 d in the experimental group were lower than the control group(P<0.05);The incidence of complications in the group 3.33%was lower than that in the control group 23.33%(P<0.05);there was no statistically significant difference between the residual rate of myomas and the recurrence rate of fibroids in the experimental group and the control group(P>0.05).Conclusion The effect of laparoscopic surgery on patients with uterine fibroids and diabetes is similar to that of open surgery,but laparoscopic surgery has less trauma,and the patients recover faster and have higher safety.
