平片式无张力疝修补术对合并腹股沟疝的老年糖尿病患者的疗效观察

Observation of Curative Effect of Plain Tension-free Hernia Repair on Elderly Diabetic Patients with Inguinal Hernia

目的观察并比较平片式无张力疝修补术和疝环充填式无张力疝修补术对合并腹股沟疝老年糖尿病患者的临床疗效。方法2012年1月—2018年1月间选择合并腹股沟疝的老年糖尿病患者160例,将其分为试验组和对照组:试验组予以平片式无张力疝修补术;对照组予以疝环充填式无张力疝修补术。比较两组的手术时长、术中出血量、住院时长、围术期血糖、术后并发症、复发情况和临床疗效。结果试验组患者的手术时长、术中出血量、住院时长、拔管即刻、术后1 h的空腹血糖、并发症发生率均显著低于对照组,差异有统计学意义(P<0.05);总有效率显著高于对照组,差异有统计学意义(P<0.05);两组术后1年内复发率差异无统计学意义(P>0.05)。结论平片式无张力疝修补术对合并腹股沟疝的老年糖尿病患者造成的创伤小、患者血糖稳定、恢复快,并发症发生率低,总体临床疗效优于充填式张力疝修补术。

Objective To observe and compare the clinical efficacy of plain tension-free hernia repair and hernia ring-filled tension-free hernia repair in elderly diabetic patients with inguinal hernia.Methods From January 2012 to January 2018,160 elderly diabetic patients with inguinal hernia were selected and divided into test group and control group:the test group received flat-sheet tension-free hernia repair;the control group received hernia ring-filled tension-free hernia repair.The operation time,intraoperative blood loss,length of hospitalization,perioperative blood glucose,postoperative complications,recurrence and clinical efficacy were compared between the two groups.Results The operation time,intraoperative blood loss,length of hospitalization,immediate extubation,fasting blood glucose at 1h postoperatively,and the incidence of complications in the experimental group were significantly lower than those in the control group,the difference was statistically significant(P<0.05);the total effective rate was significantly higher than that in the control group,the difference was statistically significant(P<0.05);There was no statistically significant difference in recurrence rate between the two groups within 1 year(P>0.05).Conclusion Plain tension-free hernia repair has little trauma,stable blood glucose,fast recovery,and low complication rate in elderly diabetic patients with inguinal hernia.The overall clinical efficacy is better than that of filled tension hernia repair.
