彩色多普勒超声在肝细胞癌与胆管细胞癌诊断及鉴别诊断中的临床应用

Color Doppler Ultrasound in Hepatocellular Carcinoma and Cholangiocarcinoma Clinical Application in Diagnosis and Differential Diagnosis

目的:分析彩色多普勒超声在肝细胞癌与胆管细胞癌诊断及鉴别中的临床应用价值。方法:选取凌海市人民医院2015年8月~2019年7月收治的40例肝细胞癌患者与40例胆管细胞癌患者开展研究,所有患者于病理检查确诊前,均开展彩色多普勒超声检查,对比两组患者的彩色多普勒超声检查结果与鉴别诊断结果。结果:通过肿瘤位置、数目、大小及超声影像表现相比,两种肿瘤具有一定差异(P<0.05);肝左叶占比、肿瘤单发占比胆管细胞癌均高于肝细胞癌,且肝硬化发生率与肝炎发生率肝细胞癌患者明显高于胆管细胞癌(P<0.05);肝细胞癌内部血流无血流低于胆管细胞癌、高速动脉血流高于胆管细胞癌,且周边血流中的高速动脉血流高于胆管细胞癌(P<0.05)。结论:在肝细胞癌与胆管细胞癌诊断及鉴别中,彩色多普勒超声应用效果显著,可通过肿瘤形态及血流情况,对病灶进行鉴别。

Objective:To analyze the clinical application value of color Doppler ultrasound in the diagnosis and differentiation of hepatocellular carcinoma and cholangiocarcinoma.Methods:40 patients with hepatocellular carcinoma and 40 patients with cholangiocarcinoma admitted to Linghai People’s Hospital from August 2015 to July 2019 were selected for research.All patients were diagnosed with color Doppler ultrasound before pathological examination.Check and compare the results of color Doppler ultrasonography and differential diagnosis of the two groups of patients.Results:Compared with the tumor location,number,size and ultrasound imaging,the two tumors have certain differences(P<0.05);the proportion of the left lobe of the liver and the proportion of single tumors in cholangiocarcinoma are higher than those of hepatocellular carcinoma,and the incidence of liver cirrhosis and hepatitis in patients with hepatocellular carcinoma was significantly higher than that of cholangiocarcinoma(P<0.05);no blood flow in hepatocellular carcinoma was lower than cholangiocarcinoma,and high-speed arterial blood flow was higher than cholangiocarcinoma,and The high-speed arterial blood flow in the peripheral blood flow was higher than that in cholangiocarcinoma(P<0.05).Conclusion:In the diagnosis and differentiation of hepatocellular carcinoma and cholangiocarcinoma,color Doppler ultrasound has a significant effect.The lesion can be differentiated by tumor morphology and blood flow.
