全膝关节置换术对比关节镜下清理术在膝关节骨性关节炎患者中的临床研究

A Clinical Study of Total Knee Replacement Surgery and Arthroscopic Debridement in Patients with Knee Osteoarthritis

目的:探讨全膝关节置换术对比关节镜下清理术在膝关节骨性关节炎患者治疗中的近期疗效。方法:选取2016年3月~2018年4月本院收治的膝骨性关节炎患者86例,依据已完成的治疗方式分为关节镜组:44例已进行膝关节镜清理术的患者;关节置换组:42例实施关节置换的患者,观察两组患者治疗前、后患者膝关节疼痛、功能等指标计算优良率并对比分析。结果:两组患者治疗前HSS膝关节功能评分差异无统计学意义,治疗后关节置换组HSS评分(90±8.28),其中优27例,良12例,可2例,差1例,优良率(92.9%)显著高于关节镜组HSS评分(81±7.32),优24例,良12例,可6例,差2例,优良率(81.8%),P<0.05。结论:全膝关节置换术对于膝骨性关节炎患者疗效确切,患者术后膝关节功能恢复、疼痛感受均优于关节镜膝关节清理术,是一般状况良好的膝骨性关节炎患者首选治疗方式。

Objective:To investigate the short-term efficacy of total knee arthroplasty compared with arthroscopic debridement in the treatment of knee osteoarthritis.Methods:86 patients with knee osteoarthritis in our hospital from March 2016 to April 2018 were selected and divided into arthroscopic group(n=44)who had undergone arthroscopic debridement;and joint replacement group(n=42)with joint replacement.Results:There was no significant difference in HSS knee function score between the two groups before treatment.After treatment,the HSS score of joint replacement group was(90±8.28),in which 27 cases were excellent,12 cases were good,2 cases were fair,and 1 case was poor.The excellent and good rate(92.9%)was significantly higher than that of arthroscopy group(81±7.32),24 cases were excellent,12 cases were good,6 cases were fair,2 cases were poor,the excellent and good rate was 81.8%,P<0.05.Conclusion:Total knee arthroplasty is effective for patients with knee osteoarthritis,and the postoperative recovery of knee joint function and pain feeling are better than arthroscopic knee debridement,which is the preferred treatment for patients with knee osteoarthritis in general good condition.
