基于MDT理念的一体化管理对缓解期双相情感障碍患者认知与社会功能康复的影响

Influence of MDT-based Integrated Management on Cognitive and Social Function Rehabilitation of Patients with Bipolar Disorder in Remission Stage

目的:探讨基于多学科协作诊疗(MDT)理念的一体化管理对缓解期双相情感障碍(BD)患者认知与社会功能康复的影响。方法:2018年8月1日~2020年2月1日,将92例缓解期BD患者采用随机数字表法分为观察组和对照组各46例,对照组进行常规干预,观察组在常规干预基础上给予基于MDT理念的一体化管理干预;比较两组干预前后认知功能[采用连线测试(TMT)、数字广度测验、威斯康星卡片分类测验(WCST)]、社会功能[采用社会功能缺陷筛选表(SDSS)]、病耻感[采用贬低-歧视感知量表(PDD)]和生活质量[采用生活质量综合评定问卷(GQOLI-74)]。结果:干预12周后,两组TMT、SDSS、PDD、GQOLI-74评分均优于干预前(P<0.05),且观察组优于对照组(P<0.01);干预12周后,观察组数字广度评分及WCST错误率均优于干预前和对照组(P<0.05)。结论:基于MDT理念的一体化管理在缓解期BD患者中实施效果良好,能够提高患者认知能力与社会功能康复效果。

Objective:To explore the effect of integrated management based on the concept of multidisciplinary treatment(MDT)on cognitive and social function rehabilitation of patients with bipolar disorder in remission.Methods:From August 1,2018 to February 1,2020,92 patients with BD in remission were divided into observation group and control group by random number table method with 46 patients in each group.The control group received routine intervention,and the observation group was given integrated management intervention based on the MDT concept.The cognitive function[Trail Making Test(TMT),Digit span test,Wisconsin card sorting test(WCST)],social function[Social Function Defect Screening Scale(SDSS)],disease shame[Perceived Devaluation and Discrimination Scale(PDD)],and the quality of life[Generic Quality of Life Inventory-74(GQOLI-74)]were compared between the two groups before and after the intervention.Results:After 12 weeks of intervention,TMT,SDSS,PDD and GQOLI-74 scores of the two groups were all better than those before intervention(P<0.05),and the observation group was better than the control group(P<0.01).After 12 weeks of intervention,the digital span test score and WCST error rate of the observation group were both better than those before intervention and better than those of the control group(P<0.05).Conclusion:Integrated management based on MDT concept has a good effect in BD patients in remission stage.It can improve the rehabilitation effects of cognitive ability and social function of patients.
