不同孕周施行宫颈环扎术治疗宫颈机能不全的效果观察

Effect of Cervical Cerclage in Different Gestational Weeks on Cervical Insufficiency

目的:观察不同孕周宫颈机能不全行宫颈环扎术的临床效果。方法:选取2018年1月~2019年12月某院收治的125例宫颈机能不全孕妇作为研究对象,根据孕周的不同将患者分为4组,所有患者均行宫颈环扎术治疗,比较不同孕周患者手术疗效以及不同手术时机手术治疗效果。结果:不同孕周患者经过手术治疗分娩孕周差异无统计学意义(P>0.05);孕周<14周患者胎儿存活率显著低于≥14周患者,流产率比较<14周患者达到32.0%,高于其他孕周患者(P<0.05),有统计学意义;孕22~28周早产率达到45.8%,高于其他孕周组(P<0.05),有统计学意义;不同手术时机疗效比较急诊手术组胎儿存活率、足月妊娠率最低,分别为89.1%、64.1%,3组比较差异有统计学意义。结论:孕14~18周是宫颈机能不全孕妇行宫颈环扎术的最佳时机,择期行宫颈环扎术效果显著,应予以重视。

Objective:To observe the clinical effect of cervical cerclage in different gestational weeks.Methods:125 pregnant women with cervical insufficiency who were admitted to a hospital from January 2018 to December 2019 were selected as the research objects.According to the different gestational weeks,the patients were divided into four groups.All patients were treated with cervical cerclage,and the results of surgical treatment in different gestational weeks were compared.Results:There was no significant difference in delivery weeks of patients at the different gestational weeks after operation(P>0.05).The fetal survival rate of the patients with gestational weeks<14 weeks was significantly lower than that of the patients with gestational weeks≥14 weeks,and the abortion rate of the patients with gestational weeks<14 weeks was 32.0%,higher than that of the patients with other gestational weeks,with statistical significance(P<0.05).The preterm birth rate of the patients with gestational weeks 22~28 was 45.8%,higher than that of the patients with other gestational weeks(P<0.05).The survival rate and full-term pregnancy rate of the emergency operation group were the lowest(89.1%and 64.1%,respectively).The difference among the three groups was statistically significant.Conclusion:14~18 weeks of gestation is the best time for cervical cerclage in pregnant women with cervical insufficiency.The effect of selective cerclage is significant and should be paid attention to.
