跟骨牵引联合内固定术对胫腓骨远端骨折患者踝关节功能的影响 被引量:3

Effect of calcaneal traction combined with internal fixation on ankle joint function in patients with distal tibial and fibular fracture

摘要 目的:探讨跟骨牵引联合内固定术对胫腓骨远端骨折患者踝关节功能的影响。方法:选取某院就诊的胫腓骨远端骨折患者87例,依据随机数表法分为对照组43例,观察组44例。所有患者均接受经皮锁定钢板内固定术治疗,对照组使用常规石膏外固定,观察组接受跟骨牵引。手术后对比2组患者手术时间、术中出血量、骨折愈合时间、踝关节及膝关节功能恢复情况。结果:2组患者骨折愈合时间比较,差异无统计学意义(P>0.05);与对照组比较,观察组手术时间较短,术中出血量较少(P<0.05);观察组膝关节功能优良率为95.45%高于对照组的79.07%(P<0.05);观察组踝关节功能优良率略高于对照组,并发症发生率略低于对照组,但差异均无统计学意义(P>0.05)。结论:针对胫腓骨远端骨折患者实施跟骨牵引联合内固定术治疗,手术时间短,创伤小,可有效改善患者的踝关节与膝关节功能,安全性较高,值得临床推广。 Objective:To investigate the effect of calcaneal traction combined with internal fixation on ankle joint function in patients with distal tibial and fibular fracture.Methods:87 patients with distal tibial and fibular fracture were randomly divided into a control group(43 cases)and an observation group(44 cases).All the patients underwent percutaneous locking plate internal fixation,with the control group undergoing routine plaster external fixation and the observation group being given calcaneal traction treatment.After surgery,the operation time,intraoperative bleeding volume,fracture healing time,recovery of ankle and knee joint function were compared between the two groups.Results:There was no statistical difference in the fracture healing time between the two groups(P>0.05).Compared with the control group,the operation time was shorter and the intraoperative bleeding volume was less in the observation group(P<0.05).The excellent rate of knee joint function in the observation group(95.45%)was higher than that in the control group(79.07%)(P<0.05).The excellent rate of ankle joint function in the observation group was slightly higher than that in the control group,and the incidence of complications in observation group was slightly lower than that in the control group,the difference being not statistically significant(P>0.05).Conclusion:Calcaneal traction combined with internal fixation in treatment of patients with distal tibial and fibular fracture has advantages of short operation time and small trauma.It can improve ankle joint function and knee joint function,and has higher safety,which is worthy of clinical promotion.
