某院2018—2019年孕中期唐氏筛查结果分析 被引量:1

摘要 目的探讨该院2019年唐氏儿患病率较2018年增高的原因,分析异常妊娠结局患者在唐氏筛查结果中不同风险区的差异,防止不良妊娠结局的产生。方法选取2017年10月至2019年的9月在该院建档的3027例孕妇作为研究对象,统计分析孕中期(2018-2019年)三联法唐氏筛查及随访数据。结果2018-2019年该院孕中期唐氏筛查的随访率为88.24%(2671/3027),唐氏筛查高风险率为5.17%(138/2671),检出率为66.67%(4/6)。妊娠结局中异常妊娠比例为3.11%(83/2671)。2019年唐氏综合征(DS)患病率与2018年比较,差异有统计学意义(P<0.05),可能原因为孕妇的年龄、孕周、户籍等不同。异常妊娠结局率在DS极高风险区、高风险区、灰区及低风险区分布差异有统计学意义(P<0.05),异常妊娠结局率在开放性脊柱裂(ONTDs)和18-三体综合征(ES)高风险区和低风险区分布差异有统计学意义(P<0.05)。结论2019年DS的患病率升高与筛查人群的不同有关,DS极高风险区、高风险区、灰区及ONTDs高风险区和ES高风险区的病例胎儿的不良妊娠结局率较高。 Objective To explore the reasons for the increase in the prevalence of Down′s syndrome(DS)in 2019 compared with 2018 in a hospital,and to analyze the differences among different risk areas in the screening results of DS among patients with abnormal pregnancy outcomes,so as to prevent the occurrence of adverse pregnancy outcomes.Methods A total of 3027 pregnant women who were registered in a hospital from October 2017 to September 2019 were selected as research objects,and the data of triad screening and follow-up in the second trimester of pregnancy(2018-2019)were statistically analyzed.Results The follow-up rate was 88.24%(2671/3027)in DS screening in second trimester,the positive rate of DS screening was 5.17%(138/2671),and the detection rate was 66.67%(4/6).The proportion of abnormal pregnancy was 3.11%(83/2671).The prevalence rate of DS in 2019 significantly different from that in 2018(P<0.05),and the possible reasons are different in age,gestational week and household registration of pregnant women.There were statistically significant differences in the distribution of abnormal pregnancy outcome rates in the high-risk,high-risk,gray and low-risk areas of DS(P<0.05),and in the distribution of abnormal pregnancy outcome rates in the high-risk and low-risk areas of ONTDs and ES(P<0.05).Conclusion The increased prevalence of DS in 2019 was associated with different screening populations,and the incidence of adverse pregnancy outcomes was higher in DS high-risk area,high-risk area,gray area,ONTDs high-risk area and ES high-risk area.
