老年急性心肌梗死PCI术后复发的危险因素调查及其预防护理对策

Investigation of risk factors for recurrence of acute myocardial infarction in elderly patients after PCI and their preventive nursing strategies

目的探讨老年急性心肌梗死PCI术后复发的危险因素及其预防护理对策。方法选择我院2017年8月至2019年8月收治的老年急性心肌梗死PCI术患者100例为研究对象,收集一般资料,统计术后复发心肌梗死患者的例数,采用多因素logistic回归分析确定老年急性心肌梗死PCI术后复发的危险因素,并制定防控护理措施。结果100例急性心肌梗死PCI术患者中,PCI术后复发心肌梗死18例,发生率为18%;多因素logistic回归分析显示,合并基础疾病、吸烟史、VAS≥4分、Killp II^IV级、术前LVEF≥40%、多支病变、服药不依从是术后复发的危险因素(P<0.05)。结论老年急性心肌梗死PCI术后复发风险有合并基础疾病、吸烟史、VAS≥4分、Killp II^IV级、术前LVEF≥40%、多支病变、服药不依从,需强化临床护理干预,采取相应防控措施。

Objective To explore the risk factors for recurrence of acute myocardial infarction in elderly patients after PCI and their preventive nursing strategies.Methods To select 100 cases of elderly patients with acute myocardial infarction who underwent PCI and admitted to our hospital from August 2017 to August 2019 as the study objects,general data was collected,and the number of patients with recurrent myocardial infarction after operation was counted,and multivariate logistic regression analysis was used to determine the risk factors for recurrence of the elderly patients with acute myocardial infarction after PCI,and formulate prevention and control nursing measures.Results Among 100 cases of patients with acute myocardial infarction undergoing PCI,18 cases had recurrent myocardial infarction after PCI,the incidence was 18%.Multivariate logistic regression analysis showed that the underlying diseases,smoking history,VAS≥4 points,Killp II^IV levels,preoperative LVEF≥40%,multivessel disease and non-compliance with medication were the risk factors for postoperative recurrence(P<0.05).Conclusion The risks of recurrence after PCI in elderly patients with acute myocardial infarction was associated with underlying diseases,smoking history,VAS≥4 points,Killp II^IV levels,preoperative LVEF≥40%,multivessel disease and non-compliance with medication.It was necessary to strengthen clinical nursing intervention and take corresponding prevention and control measures.
