采用消化内镜治疗非静脉曲张性上消化道出血的临床有效性分析

Analysis of the Clinical Effectiveness of Digestive Endoscopy in the Treatment of Non-varices Upper Gastrointestinal Bleeding

目的了解采用消化内镜治疗非静脉曲张性上消化道出血的临床有效性。方法选取2015年11月-2018年12月收治的60例非静脉曲张性上消化道出血患者随机数字表法分组。常规内科治疗组进行常规内科治疗,消化内镜手术治疗组进行消化内镜手术治疗。分析比较两组效果;非静脉曲张性上消化道出血持续的时间、血压心率恢复正常的时间、黑便呕血消失时间、引流液恢复正常时间;治疗前后患者血红蛋白监测水平和生活质量评分;复发率。结果消化内镜手术治疗组手术效果更高(χ^2=10.588,P<0.05)。常规内科治疗组总有效率70.00%(21/30),消化内镜手术治疗组100.00%(30/30)。治疗后消化内镜手术治疗组血红蛋白监测水平和生活质量评分优于常规内科治疗组(P<0.05)。消化内镜手术治疗组非静脉曲张性上消化道出血持续的时间、血压心率恢复正常的时间、黑便呕血消失时间、引流液恢复正常时间为(24.21±2.24)h、(2.01±0.22)d、(18.02±0.21)h、(2.01±0.12)d,优于常规内科治疗组(t=8.250、10.920、56.220、12.820,P<0.05)。消化内镜手术治疗组复发率低于常规内科治疗组(χ^2=6.405,P<0.05)。结论非静脉曲张性上消化道出血患者行消化内镜手术治疗效果确切,可减少出血,加速止血,降低再发生率,缩短治疗的时间,促使血红蛋白恢复正常,改善患者生活质量。

Objective To understand the clinical effectiveness of digestive endoscopy in the treatment of non-varices upper gastrointesti-nal bleeding.Methods A random number table of 60 patients with non-varices upper gastrointestinal bleeding admitted from Novem-ber 2015 to December 2018 was collected.The conventional medical treatment group received conventional medical treatment,and the digestive endoscopic surgery group received digestive endoscopic surgery.To analyze and compare the effects of the two groups;the duration of non-varices upper gastrointestinal bleeding,the time to return to normal blood pressure and heart rate,the disappear-ance of melena and hematemesis,and the time to return to normal drainage;the patient’s hemoglobin monitoring level and quality of life score before and after treatment;recurrence rate.Results The surgical effect of the digestive endoscopic surgery group was higher(χ^2=10.588,P<0.05).The total effective rate was 70.00%(21/30)in the routine medical treatment group and 100.00%(30/30)in the digestive endoscopic surgery group.After treatment,the hemoglobin monitoring level and quality of life scores of the digestive endo-scopic surgery group were better than those of the conventional medical treatment group(P<0.05).The duration of non-varices upper gastrointestinal bleeding in the digestive endoscopic surgery group,the time for blood pressure and heart rate to return to normal,the disappearance of melena and hematemesis,and the time for drainage to return to normal(24.21±2.24)h,(2.01±0.22)d,(18.02±0.21)h,(2.01±0.12)d were better than the conventional medical treatment group(t=8.250,10.920,56.220,12.820,P<0.05).The recurrence rate of the digestive endoscopic surgery group was lower than that of the conventional medical treatment group(χ^2=6.405,P<0.05).Conclusion Digestive endoscopic surgery for patients with non-varices upper gastrointestinal bleeding has a definite effect,which can reduce bleeding,accelerate hemostasis,reduce recurrence rate,shorten treatment time,promote hemoglobin to normal,and improve the quality of life of patients.
