甲根上局部浸润麻醉在拇趾甲沟炎拔甲术中的应用效果

The effect of the application of local infiltration anesthesia on nail root to nail extraction for great toe paronychia

目的分析甲根上局部浸润麻醉在拇趾甲沟炎拔甲术中的应用效果。方法选取2017年6月~2019年4月我院收治的60例拇趾甲沟炎患者,其均接受拇趾甲沟炎拔甲术治疗,随机分为对照组和实验组,各30例,实验组患者采用甲根上局部浸润麻醉进行麻醉,对照组患者采用趾根神经阻滞麻醉进行麻醉,比较两组患者的麻醉时间、麻醉剂用量、麻醉效果、拔甲术全操作时长和满意度等指标。结果在本研究中,两组患者手术过程的VAS评分和注药时间差异无统计学意义(P>0.05)。实验组患者的拔甲术术中时间和麻醉时间显著短于对照组患者,且麻醉剂用量显著少于对照组患者,差异有统计学意义(P<0.05)。实验组患者的满意度为100.00%,而对照组患者的满意度为76.67%,差异有统计学意义(P<0.05),实验组患者并发症发生率为3.33%,对照组患者并发症发生率为26.67%,差异有统计学意义(P<0.05)。结论拇趾甲沟炎患者接受拇趾甲沟炎拔甲术治疗时,采用甲根上局部浸润麻醉进行麻醉的应用效果更加显著,拔甲术全程操作时间和麻醉时间更短,麻醉剂用量也更小,值得广泛用于临床实践中。

Objective To analyze the effect of the application of local infiltration anesthesia on nail root to nail extraction for great toe paronychia.Methods 60 patients with great toe paronychia admitted to and treated in our hospital from June 2017 to April 2019 were selected and all treated with nail extraction for great toe paronychia.They were randomly divided into the control group and the experimental group,with 30 patients in each group.The patients of the experimental group were anesthetized with local infiltration anesthesia on nail root,while the patients of the control group were anesthetized with nerve blocking anesthesia of toe root.The indexes such as anesthesia duration,anesthetic dosage,anesthesia effect,total operation time of nail extraction and satisfaction were compared between the two groups of patients.Results In this study,the differences of the VAS scores during operation and infusion time between the two groups of patients were statistically insignificant(P>0.05).The operation duration and anesthesia duration of nail extraction for the patients of the experimental group were significantly shorter than those of the control group,and the anesthetic dosage was significantly less than that of the control group,with significant differences between the two groups(P<0.05).The satisfaction of the patients in the experimental group was 100.00%,while that in the control group was 76.67%,with significant difference between the two groups(P<0.05).The incidence of complications among the patients of the experimental group was 3.33%,while that of the control group was 26.67%,with significant difference between the two groups(P<0.05).Conclusion When the patients with great toe paronychia were treated with nail extraction for great toe paronychia,the anesthetic scheme of local infiltration anesthesia on nail root has more obvious effect of application.The overall operation duration and anesthesia duration of the nail extraction are shorter,and the anesthetic dosage is less.Therefore,it is worthy of wide application in clinical practice.
