玻璃体切割联合内界膜剥离术治疗黄斑裂孔性视网膜脱离临床研究

Clinical Study of Pars Plana Vitrectomy Combined with Internal Limiting Membrane Dissection for Macular Hole Retinal Detachment

目的探索玻璃体切割联合内界膜剥离术对治疗黄斑裂孔性视网膜脱落患者的效果。方法选取本院2015年8月—2019年8月收治的90例黄斑裂孔性视网膜脱离患者为研究对象,以区组随机化方法,并按相同比例将其分为参照组与试验组,每组45例。参照组患者均行玻璃体切割治疗,试验组患者则在玻璃体切割治疗基础上,联合使用内界膜剥离术治疗。观察术后视网膜复位率及治疗3、6个月后患者视力情况。结果试验组患者术后的视网膜复位率明显高于参照组,差异有统计学意义(P <0.05);治疗3、6个月后,试验组患者的最佳矫正视力均明显高于参照组,差异有统计学意义(P <0.05);试验组患者的视力情况改善率为71.11%,显著高于参照组的48.89%,差异有统计学意义(P <0.05)。结论以玻璃体切割联合内界膜剥离术对治疗黄斑裂孔性视网膜脱落具有较高的安全性,患者恢复效果好。

Objective To explore the related research of vitrectomy combined with internal limiting membrane dissection for the treatment of patients with macular hole retinal detachment.Methods 90 patients with macular hole retinal detachment treated in our hospital from August 2015 to August 2019 were selected as the study subjects.They were randomized by block method and divided into reference group and test group in the same proportion,45 cases in each group.Patients in the reference group were treated with vitrectomy,while patients in the test group were treated with internal limiting membrane dissection on the basis of vitrectomy.Observed the postoperative retinal reduction rate,the best corrected visual acuity of the patients after 3 and 6 months and the visual acuity improvement rate.Results Postoperatively,the retinal reduction rate of patients in the test group was significantly higher than that in the reference group,and the results had significant statistical differences(P<0.05).After 3 and 6 months of treatment,the best corrected visual acuity of patients in the test group was significantly higher than that in the reference group,and the results had significant statistical differences(P<0.05);the visual acuity improvement rate of the test group was 71.11%,which was significantly higher than 48.89%of the reference group,the difference was statistically significant(P<0.05).Conclusion Vitreous cutting combined with internal limiting membrane dissection has high safety in treating macular hole retinal detachment,and the patient has good recovery effect.
