加味地黄汤治疗肝肾阴虚型老年高血压病的临床研究

Clinical Study of Jiawei Dihuang Decoction in the Treatment of Senile Hypertension With Yin Deficiency of Liver and Kidney

目的探讨加味地黄汤治疗肝肾阴虚型老年高血压病的临床疗效及对患者动态血压及血浆肾素水平的影响。方法将2018年3—12月符合纳入标准的100例肝肾阴虚型老年高血压病病例随机分对照组和治疗组,各50例。对照组给予非洛地平,治疗组在此基础上给予加味地黄汤,两组疗程均为4周。观察两组治疗前后24 h动态血压、血浆肾素水平的变化及治疗前后血压、症状改善情况。结果 (1)治疗组降压疗效的显效率及症状疗效明显优于对照组,差异有统计学意义(P<0.05);(2)两组治疗后24 h动态血压与治疗前比较,差异均有统计学意义(P <0.01),且治疗组低于对照组,差异有统计学意义(P<0.05);(3)治疗后治疗组血浆肾素水平较对照组低,差异有统计学意义(P <0.05)。结论加味地黄汤治疗肝肾阴虚型老年高血压病疗效显著,不仅能明显改善患者的临床症状,并通过降低血浆肾素水平而发挥降压作用,值得在临床中应用及推广。

Objective To explore the clinical effect of Modified Dihuang Decoction in the treatment of elderly hypertension with Yin deficiency of liver and kidney and its influences of patients with dynamic blood pressure and plasma renin level.Methods From March to December 2018,100 cases of elderly hypertension with Yin deficiency of liver and kidney were randomly divided into control group and treatment group with 50 cases each.First,the control group was given felodipine,and the treatment group was given Modified Dihuang Decoction.Both groups were treated for four weeks.The changes of blood pressure,symptoms,twenty-four hours dynamic blood pressure and plasma renin level were observed before and after treatment.Results(1)The apparent efficiency of antihypertensive effect and symptomatic effect of the treatment group were significantly better than those of the control group,and the differences were statistically significant(P<0.05).(2)The twenty-four hours dynamic blood pressure of the two groups after treatment were significantly different from those before treatment,the differences were statistically sginificant(P<0.01);and the indicators in the treatment greoup were lower than those of the control group,the differences were statistically significant(P<0.05).(3)The plasma renin level in the treated group after treatment was significantly lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion Modified Dihuang Decoction has a significant effect on elderly hypertension with yin deficiency of liver and kidney.It can not only obviously improve the clinical symptoms of patients,but also exert antihypertensive effect by lowering plasma renin level.It is worthy of clinical application and promotion.
