摘要
目的分析研讨脑损伤早产儿的头颅MR检查结果及导致新生儿脑损伤的高危因素。方法随机从该院2016年12月—2018年12月期间收治的早产儿中抽取70名进行讨论,均接受头颅MR检查,分析检查结果以及发生新生儿脑损伤的高危因素。结果70名早产儿中,共32名发生脑损伤(45.71%),包含10名脑室周围白质软化和22名脑室内出血患儿,另38名早产儿未发生脑损伤(54.29%)。脑损伤组母体感染率46.88%、低氧血症率81.25%、呼吸窘迫综合征率75.00%高于对照组5.26%、26.32%、28.95%禾出生5 min Apgar评分(4.68±0.56)分、出生体质量(1567.23±230.25)g、胎龄(31.01±0.23)周低于对照组(7.35±0.52)分、(2569.58±461.25)g、(33.68±0.15)周,数据差异有统计学意义(P<0.05),多因素Logistic回归分析得知,母体感染、低氧血症、呼吸窘迫综合征、出生5 min Apgar评分、出生体质量、胎龄均为影响新生儿脑损伤的危险因素。结论临床确诊脑损伤早产儿可采用头颅MR检查,此方式具有一定应用价值。母体感染、低氧血症、呼吸窘迫综合征、出生5 minApgar评分低、出生低体质量、胎龄小等均为引发新生儿脑损伤的主要高危因素。
Objective To analyze the brain MR examination results of premature infants with brain injury and the high-risk factors leading to neonatal brain injury.Methods Seventy preterm infants randomly selected from the hospital from December 2016 to December 2018 were selected for discussion.All of them underwent head MR examination,and analyzed the examination results and the high-risk factors of neonatal brain injury.Results Of the 70 preterm infants,a total of 32 cases had brain injury(45.71%),including 10 cases of periventricular white matter softening and 22 cases of intraventricular hemorrhage,and 38 cases of premature infants had no brain injury(54.29%).The maternal infection rate in the brain injury group was 46.88%,the rate of hypoxemia was 81.25%,the rate of respiratory distress syndrome was 75.00%higher than the control group 5.26%,26.32%,28.95%,5 min Apgar score(4.68±0.56)points at birth,birth weight(1567.23±230.25)g,gestational age(31.01±0.23)weeks were lower than the control group(7.35±0.52)points,(2569.58±461.25)g,(33.68±0.15)weeks,the data difference was statistically significant(P<0.05),multivariate logistic regression analysis revealed that maternal infection,hypoxemia,respiratory distress syndrome,5 min Apgar score at birth,birth weight,and gestational age are all risk factors that affect neonatal brain injury.Conclusion The cranial MR examination can be used in clinically diagnosed preterm infants with brain injury.This method has certain application value.Maternal infection,hypoxemia,respiratory distress syndrome,low 5 min Apgar score at birth,low birth weight,and small gestational age are the main high-risk factors that cause neonatal brain injury.
