偏瘫上肢七级功能训练配合醒脑开窍针法治疗脑卒中后上肢功能障碍的疗效研究

A Study on the Therapeutic Effect of Upper-limb Functional Training of Hemiplegia Combined with Xingnao Kaiqiao Acupuncture on Upper Limb Dysfunction after Stroke

目的观察偏瘫上肢七级功能训练结合醒脑开窍对脑卒中后得上肢功能恢复的临床疗效。方法将深圳市宝安中医院(集团)针灸医院2018年1月—2019年6月收治的70例脑卒中上肢功能障碍的偏瘫患者进行临床观察,根据随机对照,分为观察组34例和对照组36例,两组性别、年龄、病程、卒中类型、偏瘫部位等比较差异无统计学意义(P>0.05)。两组皆进行常规康复训练,观察组在常规康复训练基础上增加偏瘫上肢七级功能训练及醒脑开窍针法治疗。两组患者每周进行6次治疗,4周为一个疗程,共治疗1个疗程,治疗前后两组分别进行偏瘫上肢七级功能评估、Fug1-Meyer量表上肢部分(Fug1-Meyer assessment of upper extremity,FMA-UE)、改良Barthel指数评分(Modified Barthel index,MBI),健康调查简表(Medical outcomes study(MOS)36-item short form health survey,SF-36)评定两组疗效。结果两组患者治疗4周后,其偏瘫上肢七级功能评估、FMA-UE、MBI、SF-36评分与治疗前相比较差异具有显著性意义(P<0.05),且观察组优于对照组(P<0.05)。结论将偏瘫上肢七级功能训练结合醒脑开窍针法治疗脑卒中患者,可促进上肢功能恢复,提高日常生活能力,从而提升生活质量。

Objective To observe the clinical effect of hemiplegic upper limb function training combined with Xingnao Kaiqiao on upper limb function recovery after stroke.Methods 70 cases of hemiplegia patients with stroke and upper limb dysfunction admitted to Shenzhen Baoan Hospital of Traditional Chinese Medicine(Group)Acupuncture and Moxibustion Hospital from January 2018 to June 2019 were divided into observation group and control group according to random control.In 36 cases,there was no statistically significant difference between the two groups in terms of gender,age,course of disease,stroke type,and hemiplegic location(P>0.05).Both groups were given routine rehabilitation training,and the observation group added seven-level functional training for hemiplegia upper limbs and Xingnao Kaiqiao acupuncture treatment on the basis of conventional rehabilitation training.The two groups of patients were treated 6 times a week,4 weeks as a course of treatment,a total of one course of treatment,before and after treatment,the two groups were respectively subjected to hemiplegic upper extremity function evaluation,Fug1-Meyer assessment of upper extremity(Fug1-Meyer assessment of upper extremity,FMA-UE),Modified Barthel index(MBI),Medical outcomes study(MOS)36-item short form health survey(SF-36)to evaluate the efficacy of the two groups.Results After 4 weeks of treatment,the two groups of patients with hemiplegic upper limb function evaluation,FMA-UE,MBI,SF-36 scores were significantly different from those before treatment(P<0.05),and the observation group was superior to the control group(P<0.05).Conclusion Treatment of stroke patients with hemiplegic upper limb seven-level functional training combined with Xingnao Kaiqiao acupuncture treatment can promote the recovery of upper limb function,improve the ability of daily life,and thus improve the quality of life.
