吻合器痔上黏膜环切术联合外剥内扎术治疗中重度痔疮的疗效分析

Analysis of the Therapeutic Effect of Stapler Hemorrhoids Combined with External Stripping and Internal Ligation on Moderate and Severe Hemorrhoids

目的探究为中重度痔疮患者实施吻合器痔上黏膜环切手术联合外剥内扎手术治疗的临床效果。方法2017年9月—2019年11月,将该院92例中重度痔疮患者随机等分为两组,每组各46例,参照组行单纯性外剥内扎手术治疗,研究组实施吻合器痔上黏膜环切手术联合外剥内扎手术治疗,对比两组的并发症发生率、创面愈合实现时间指标、手术过程持续时间指标、住院持续时间指标、日常工作恢复时间指标,以及接受治疗前后的VAS疼痛评分指标。结果研究组的并发症发生率15.22%(7/46)低于参照组34.78%(16/46),组间差异有统计学意义(χ2=4.696,P<0.05)。研究组的创面愈合实现时间指标(6.44±1.19)d短于参照组(14.74±3.58)d,组间差异有统计学意义(t=14.884,P<0.05)。治疗后,研究组的VAS疼痛评分指标(0.24±0.09)分低于参照组(4.44±1.32)分,组间差异有统计学意义(t=21.530,P<0.05)。结论为中重度痔疮患者实施吻合器痔上黏膜环切手术联合外剥内扎手术治疗,能获取较好效果。

Objective To explore the clinical effect of combined stapler hemorrhoid periprocedectomy combined with external stripping and internal ligation for patients with moderate to severe hemorrhoids.Methods From September 2017 to November 2019,92 patients with moderate to severe hemorrhoids in the hospital were randomly divided into two groups,with 46 cases in each group.The reference group was treated with simple external stripping and internal ligation,and the study group performed anastomosis treatment of hemorrhoidal hemorrhoids combined with external stripping and internal ligation surgery,comparing the complication rate,wound healing time index,surgical process duration index,hospitalization duration index,daily work recovery index,and receiving treatment before and after the VAS pain score index.Results The incidence of complications in the study group was 15.22%(7/46)lower than that in the reference group,34.78%(16/46),and there was a statistically significant difference between the groups(χ2=4.696,P<0.05).The time to achieve wound healing in the study group(6.44±1.19)d was shorter than the reference group(14.74±3.58)d,and there was a statistically significant difference between the groups(t=14.884,P<0.05).After treatment,the VAS pain score index of the study group(0.24±0.09)points was lower than that of the reference group(4.44±1.32)points,and there were statistically significant differences between the groups(t=21.530,P<0.05).Conclusion For patients with moderate to severe hemorrhoids,anastomotic hemorrhoids combined with external stripping and internal ligation surgery can achieve good results.
