妇产科术后下肢静脉血栓的诊断与处理

Diagnosis and Management of Venous Thrombosis of Lower Extremities after Gynecology and Obstetrics

目的研究妇产科术后下肢静脉血栓的诊断与处理。方法选取在2017年1月—2019年3月来该院就诊的82例具有血栓中、高危因素并且采用妇科盆腔手术的患者为研究对象,设为观察组和对照组,每组41例。对照组:低分子肝素治疗;观察组:利伐沙班治疗。对比两组INR、D-二聚体的变化和治疗总有效率情况。结果观察组患者的INR(0.64±0.02)与对照组患者的INR(2.59±0.17)相比较低(t=3.311,P=0.035<0.05),D-二聚体(293.13±79.32)μg/mL与对照组患者的D-二聚体(552.76±96.51)μg/mL相比较低(t=13.317,P=0.003<0.05)。观察组患者的治疗总有效率97.56%与对照组82.93%相比较高(χ2=7.689,P=0.031<0.05)。结论D-二聚体能够有效的诊断出妇产科术后下肢静脉血栓,应用抗血栓梯度弹力袜以及低分子肝素能够预防术后下肢静脉血栓的发生,提高了治疗总有效率,值得在临床中应用。

Objective To study the diagnosis and treatment of venous thrombosis of lower extremities after obstetrics and gynecology.Methods A total of 82 patients with medium-and high-thrombosis factors who were treated in the hospital from January 2017 to March 2019 and used gynecological pelvic surgery were selected as the research object.They were set as the observation group and the control group,with 41 cases in each group.Control group:low-molecular heparin therapy;Compare the changes of the two groups of INR and D-dipolymer with the general efficiency of treatment.Results Inrs(0.64±0.02)in the observation group had lower INR(2.59±0.17)of the control group(t=3.311,P=0.035<0.05),D-dipolymer(293.13±79.32)μg/mL was lower than the D-dipolymer(552.76±96.51)μg/mL in the control group(t=13.317,P=0.003<0.05).The total treatment efficiency of the patients in the observation group 97.56%was higher than the control group's 82.93%(χ2=7.689,P=0.031<0.05),Conclusion D-dipolymer can effectively diagnose postoperative postoperative postoperative venous thrombosis,application of anti-gradient thrombosis and low-molecular hemogincane can prevent the occurrence of postoperative postoperative thrombosis,improve the treatment efficiency,and is worth the clinical application.
