摘要
目的探讨单颗牙微创种植后即刻负荷对种植体周围龈沟液炎性因子水平的影响。方法选取2018年7月—2019年6月收治的84例行牙齿修复的患者进行研究,根据种植方法分为研究组(n=42)与参照组(n=42),参照组行单颗牙切开翻瓣种植,研究组行单颗牙微创种植后即刻负荷,对两组种植效果进行观察。结果研究组手术时长明显短于参照组[(9.27±1.34)min vs(25.41±3.05)min],牙槽骨吸收量明显低于参照组[(0.22±0.03)mm vs(1.25±0.12)mm],差异有统计学意义(t=31.398、53.965,P<0.05);两组术后1个月、2个月TNF-α、IL-1β水平与术后1周相比均明显降低,差异有统计学意义(P<0.05);研究组术后1周、1个月、2个月TNF-α、IL-1β水平均低于参照组,差异有统计学意义(P<0.05)。结论单颗牙微创种植后即刻负荷为一种简单省时的种植术,可减少牙槽骨吸收,术后早期NF-α,IL-1β可参与骨改建的免疫调节,随炎症水平逐渐稳定可实现种植体良好骨整合,可缩短患者冠修复的时间。
Objective To investigate the effect of load on the inflammatory factor level of gingival crevicular fluid around implant after minimally invasive implantation.Methods A total of 84 patients undergoing dental restorations from July 2018 to June 2019 were selected for study.They were divided into a research group(n=42)and a reference group(n=42)according to the implantation method.The flap was opened for implantation,and the study group underwent a single tooth implant immediately after minimally invasive implantation,and observed the implantation effect of the two groups.Results The operation duration of the study group was significantly shorter than that of the reference group[(9.27±1.34)min vs(25.41±3.05)min],and the alveolar bone resorption was significantly lower than that of the reference group[(0.22±0.03)mm vs(1.25±0.12)mm](t=31.398,53.965,P<0.05),the difference was statistically significant(P<0.05);the levels of TNF-αand IL-1βat 1 month and 2 months after operation in both groups were significantly lower than that at 1 week after operation,and the difference was statistically significant(P<0.05);the levels of TNF-αand IL-1βin the study group at 1 week,1 month,and 2 months after operation were lower than those in the reference group,and the difference was statistically significant(P<0.05).Conclusion Immediate load after a minimally invasive implant is a simple and time-saving implant,which can reduce alveolar bone resorption.Early postoperative NF-αand IL-1βcan participate in the immune regulation of bone remodeling and gradually stabilize with the level of inflammation can achieve good osseointegration of the implant and shorten the time of crown repair in patients.
