综合护理干预对骨科大手术后下肢深静脉血栓的预防作用

Preventive Effect of Comprehensive Nursing Intervention on Deep Vein Thrombosis of Lower Limbs after Major Orthopedic Surgery

目的探讨骨科大手术后采用综合护理干预对预防下肢深静脉血栓的作用。方法以2018年5月—2019年12月作为时间,根据护理方法将在该院进行骨科手术88例患者进行分组,实验组与参照组,每组各44例患者,采用常规护理干预44例患者为参照组,采用综合护理干预44例患者为实验组,对比实验组与对照组患者下肢血流速度、并发症发生率以及护理满意度。结果实验组患者下肢血流速度(26.82±5.64)cm/s,参照组患者下肢血流速度(21.55±4.31)cm/s,实验组患者下肢血流速度显著优于参照组患者(t=4.925,P=0.000)。实验组患者下肢深静脉血栓发生率4.54%与下肢肿痛发生率6.18%显著低于参照组患者(χ2=8.494、10.057,P=0.004、0.002)。实验组患者护理满意度97.72%高于参照组患者72.72%(χ2=10.921,P=0.001)。结论骨科大手术后患者应用综合护理可以有效预防下肢深静脉血栓,可以有效减少DVT、下肢肿胀的发生率,进而可以提高护理满意度,具有临床使用与推广价值。

Objective To explore the effect of comprehensive nursing intervention on prevention of deep venous thrombosis of lower extremities after major orthopedic surgery.Methods From May 2018 to December 2019,88 patients underwent orthopedic surgery in the hospital were grouped according to the nursing method.The experimental group and the reference group,44 patients in each group,used routine nursing intervention;44 cases in the reference group with comprehensive nursing intervention were the experimental group.The blood flow velocity of the lower limbs,the incidence of complications and the nursing satisfaction were compared between the experimental group and the control group.Results The lower limb blood flow velocity in the experimental group was(26.82±5.64)cm/s,and the lower limb blood flow velocity in the reference group was(21.55±4.31)cm/s.The lower limb blood flow velocity in the experimental group was significantly better than that in the reference group(t=4.925,P=0.000).The incidence of lower limb deep vein thrombosis in the experimental group was 4.54%and the incidence of lower limb swelling and pain was significantly lower than that in the reference group(χ2=8.494,10.057,P=0.004,0.002).The nursing satisfaction of the experimental group was 97.72%higher than that of the reference group 72.72%(χ2=10.921,P=0.001).Conclusion Comprehensive nursing after orthopedic surgery can effectively prevent deep vein thrombosis of lower extremities,and can effectively reduce the incidence of DVT and lower extremity swelling,which can improve nursing satisfaction and has clinical use and promotion value.
