基于真实世界的痉挛性脑瘫患儿病历回顾性研究

A Retrospective Study on Medical Records of the Children with Spastic Cerebral Palsy Based on Real World

目的:通过对大样本痉挛型脑瘫患儿病历资料的数据挖掘,探讨基于真实世界的中医临床研究关于痉挛型脑瘫患儿年龄、性别、智力状况和患儿运动功能康复疗效的相关关系,为提高痉挛型脑瘫患儿的康复疗效提供科学依据。方法:采用病例回顾性研究,对西安中医脑病医院2014—2018年第一、二诊断为痉挛性脑性瘫痪患儿的病例资料进行回顾,通过基于真实世界临床研究的结构化电子病历模拟系统录入数据,分析患儿智力因素与其精神运动和社会适应能力相关性。结果:正常智力患儿康复后精细、粗大运动功能评定结果优于智力障碍重度及智力低于正常的患儿,但组间比较差异无统计学意义(P>0.05)。正常智力患儿康复后肌张力评定结果优于智力障碍重度和智力低于正常的患儿,但组间比较差异无统计学意义(P>0.05)。结论:患儿的首诊年龄和智力因素影响其康复疗效。

Objective:To explore the correlation between the age,gender and intellectual condition of the children with spastic cerebral palsy(SCP)and clinical effects of motor function rehabilitation based on the real world through unearthing the data of medical records of SCP children in large sample,to provide scientific reference for improving the rehabilitation effects of SCP children.Methods:Retrospective study of medical cases was used to review the data of the cases of the first and second diagnosis as SCP between 2014 and 2018,the data was typed into structured electronic medical record simulation system for clinical research based on real world,to analyze the correlation of intellectual factors and their psychomotor and social adaptability.Results:After rehabilitation,the fine and gross motor function of children with normal intelligence were better than those with severe mental retardation and lower intelligence than normal children,while the difference had no statistical meaning between both groups(P>0.05).After rehabilitation,the muscle tension of children with normal intelligence is better than that of children with severe mental retardation and intelligence lower than that of normal children,while the difference had no statistical meaning between both groups(P>0.05).Conclusion:Blind diagnosis age and intellectual factors could affect the children's rehabilitation effects.
