耳穴联合腕踝针结合常规疼痛护理对髋关节置换术后疼痛控制的影响

Effects of Auricular Points and Wrist-ankle Acupuncture Combined with Conventional Pain Nursing on Pain Intervention after Hip Replacement

目的:探讨耳穴联合腕踝针结合常规疼痛护理对髋关节置换术后疼痛控制的影响。方法:采用便利抽样法,将行人工髋关节置换术的髋关节炎患者118例根据镇痛方法不同分为观察组60例与对照组58例。所有患者术后采用自控静脉镇痛,对照组给予常规疼痛护理干预,观察组在对照组护理的基础上给予耳穴联合腕踝针镇痛干预,记录两组预后情况。结果:观察组患者术后3天、7天及14天的VAS评分均低于对照组(P<0.05);两组患者术后1~3天的自控静脉镇痛药物用量比较无显著差异(P>0.05),两组均未出现追加镇痛药物情况;观察组术后3天、7天及14天的血清肿瘤坏死因子α(tumor necrosis factorα,TNF-α)与白细胞介素6(interleukin-6,IL-6)含量均低于对照组(P<0.05),且高于术前1天(P<0.05);两组术后3个月髋关节Harris评分均高于术前1天(P<0.05),观察组高于对照组(P<0.05)。结论:髋关节置换术后采用耳穴联合腕踝针结合常规镇痛护理能更有效地控制疼痛,抑制血清TNF-α与IL-6释放,且不会增加自控静脉镇痛药物使用量,促进改善髋关节功能。

Objective:To explore the effects of auricular points and wrist-ankle acupuncture combined with conventional pain nursing on pain intervention after hip replacement.Methods:Using convenience sampling,118 patients who accepted total hip arthroplasty(THA)were allocated to 60 cases of the observation group and 58 cases of the control group in light of different analgesic methods.All the patients adopted patient-controlled intravenous analgesia,the control group accepted conventional pain nursing intervention,and the observation group auricular points and wrist-ankle acupuncture,to record the prognostic conditions in both groups.Results:The observation group was lower than the control group in VAS scores three,seven and fourteen days after the operation(P<0.05).There was no significant difference in the dosage of patient-controlled intravenous analgesia medicine between both groups one to three days after the operation,no additional analgesic drugs were found in both groups(P>0.05).The levels of TNF-αand IL-6 of the observation group were lower than these of the control group three,seven and fourteen days after the operation(P<0.05),higher than these one day before surgery(P<0.05),Harris hip scores of both groups three months after the surgery were higher than these one day before the surgery(P<0.05),the observation group was higher than the control group(P<0.05).Conclusion:Auricular points and wrist-ankle acupuncture combiend with conventional pain nursing could effectively control pain after hip replacement,inhibit the release of TNF-αand IL-6,and it won't increase the dosage of patient-controlled intravenous analgesia medicine,promote the improvements of hip function.
