阻塞性睡眠呼吸暂停低通气综合征对代谢的影响研究

Impact of obstructive sleep apnea-hypopnea syndrome on metabolism

目的分析阻塞性睡眠呼吸暂停低通气综合征(OSAHS),以及不同阻塞程度对代谢指标的影响。方法纳入该院近3年收治的OSAHS确诊患者51例(患者组),根据呼吸暂停低通气指数将患者分轻、中、重度组;匹配年龄、性别、身高后,选择同期体检健康者20例(对照组)。比较分析患者与健康者间,以及不同阻塞程度组患者间体重、体重指数(BMI)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、血糖、尿酸及血压水平的差异。结果患者组体重、BMI、血糖、尿酸、收缩压、舒张压、平均动脉压(MAP)水平均高于对照组(P<0.05),TC、LDL-C水平略有增高,但差异无统计学意义(P>0.05)。不同阻塞程度组与对照组血糖、LDL-C、TC水平差异无统计学意义(P>0.05),但体重、BMI、尿酸、收缩压、舒张压、MAP水平存在差异(P<0.05)。中、重度组与对照组,以及轻度组与中度组之间,体重水平差异有统计学意义(P<0.05);中、重度组与对照组,以及重度组与轻度组之间,BMI水平差异有统计学意义(P<0.05);中、重度组与对照组,以及轻度组与中度组之间,尿酸水平差异有统计学意义(P<0.05);不同阻塞程度组与对照组之间血压相关指标水平差异有统计学意义(P<0.05),但不同阻塞程度组间比较差异无统计学意义(P>0.05)。结论OSAHS患者体重、BMI、尿酸、血糖、收缩压、舒张压、MAP水平高于健康者,阻塞程度对血糖、收缩压、舒张压、MAP水平无影响;中、重度OSAHS患者体重、BMI、尿酸水平升高明显。。

Objective To investigate the impact of obstructive sleep apnea-hypopnea syndrome(OSAHS)and different obstruction degrees on metabolic indexes.Methods A total of 51 OSAHS patients treated in the past three years were enrolled(patient group)and divided into mild group,moderate group and severe group according to apnea-hypopnea index(AHI)levels.20 healthy subjects,matching the age,gender and height of patient group,were enrolled as control group.Body weight,body mass index(BMI),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),blood glucose,uric acid and blood pressure were compared between different groups.Results Levels of body weight,BMI,blood glucose,uric acid,systolic pressure,diastolic pressure and mean arterial pressure(MAP)of patient group were higher than control group(P<0.05),and levels of TC and LDL-C in patients group were higher than control group,but without significant difference(P>0.05).Levels of blood glucose,LDL-C and TC were without significant difference between different obstruction degree groups and control group(P>0.05),whereas there were significant difference of body weight,BMI,uric acid,systolic pressure,diastolic pressure and MAP(P<0.05).Compared with control group,there was significant difference of body weigh in moderate and severe group,and also was different between mild group and moderate group(P<0.05).Compared with control group,there was significant difference of BMI in moderate and severe group,and also was different between severe group and mild group(P<0.05).Compared with control group,there was significant difference of uric acid in moderate and severe group,and also was different between moderate group and mild group(P<0.05).There was significant difference of systolic pressure,diastolic pressure and MAP between different obstruction degree groups and control group(P>0.05),but among the different obstruction degree groups,there was no significant difference(P>0.05).Conclusion Levels of body weight,BMI,uric acid,blood glucose,systolic pressure,diastolic pressure and MAP of OSAHS patients could be higher than healthy subjects,and there might be no impacting effects of obstruction degree on blood glucose,systolic pressure,diastolic pressure and MAP.Levels of body weight,BMI and uric acid could be significantly increased in moderate and severe OSAHS patients.
