新疆克拉玛依市艾滋病一线药物抗病毒治疗失败病例的耐药情况及影响因素

Analysis on drug resistance and influencing factors of HIV/AIDS first-line antiviral therapy failure cases in Karamay city of Xinjiang

目的了解新疆克拉玛依市艾滋病一线药物抗病毒治疗失败病例的耐药情况,并探讨相关影响因素。方法选择艾滋病一线药物抗病毒治疗失败的病例,并进行耐药基因型检测,应用logistic回归模型分析耐药的相关危险因素。结果51例治疗失败的病例纳入研究,耐药率为51.0%;>40岁、确证阳性至开始治疗时间>2年的病例更可能发生基因型耐药(P值均<0.05)。结论应加强对中老年、确证阳性至开始治疗时间较长的HIV感染者抗病毒治疗的随访管理,提高抗病毒治疗的效果,降低耐药的发生,减少患者的死亡。

Objective To investigate the drug resistance of HIV/AIDS patients who failed to receive first-line antiviral therapy in Karamay city of Xinjiang;to explore the related influencing factors.Methods The patients who failed to receive first-line antiviral therapy were selected and genotyped for drug resistant genes.Logistic regression model was used to analyze related risk factors.Results A total of 51 patients with treatment failure were included in the study,the drug resistance rate was 51.0%;the patients over 40 years old and initiation treatment for more than 2 years after confirmation were more likely to have genotype resistance(all P<0.05).Conclusion The follow-up management of antiviral treatment for middle-aged and elderly HIV infected patients initiating treatment long time after confirmation,in order to improve the effect of antiviral treatment,reduce the occurrence of drug resistance and death of patients.
