摘要 目的运用技术难度系数校正护理工时,构建护理项目工作量校正系数库。方法通过现场调查临床护理项目开展情况筛选纳入项目并测量各项目工时;通过Delphi法专家函询护理项目技术难度,评价维度及各维度权重,并评定各项目技术难度系数,将技术难度系数乘以护理工时系数得到各项目校正系数。结果计算得到5个类别共175项护理操作项目的校正系数,最高为“抢救”项目103.36、最低为“更换饮食护理级别卡”项目0.56。结论涵盖5个类别175项护理操作项目的校正系数库将难度纳入护理工作量的测量中,改善传统护理工作量的弊端,为优化临床绩效考核制度,建立提高护士工作积极性的体制机制提供指导。 Objective To correct nursing hours by using technical difficulty coefficient,so as to construct a library of nursing workload correction coefficients.Methods In-site investigation was used to investigate the development of clinical nursing items and measur the working hours.The technical difficulty evaluation dimension,the weight of each dimension and the technical difficulty coefficient of each project was evaluated by Delphi method.Multiplying the technical difficulty coefficient by the nursing time coefficient to obtained the correction coefficient of each item.Results The correction coefficients of 175 nursing items in 5 categories were calculated.The item of"rescue"had the highest correction coefficients(103.36),and the"change the diet card"item was the lowest(0.56).Conclusion A library of correction coefficients covering 175 nursing items incorporates difficulty into the measurement of nursing workload,to improve the drawbacks of the traditional nursing workload,and to provide guidance for optimizing clinical performance appraisal system and establishing institutional mechanisms to improve the enthusiasm of nurses.
