四等级功能活动评分法在胸腰椎术后患者早期疼痛管理中的应用

Application of four grade functional activity score in early pain management of patients after thoracolumbar spine surgery

目的探讨四等级功能活动评分法在胸腰椎术后患者早期疼痛与功能锻炼管理中的应用效果。方法选取2017年1月至6月某三级甲等医院脊柱外科收治的97例胸腰椎手术患者为对照组,给予常规围手术期疼痛评估与护理以及功能锻炼指导;选取2017年7月至12月收治的95例胸腰椎手术患者为干预组,在对照组基础上成立术后活动性疼痛干预小组、建立患者疼痛护理档案,应用四等级功能活动评分法以及数字评定量表对患者术后疼痛情况进行综合评估并记录,根据评估结果指导实施功能锻炼。结果两组患者术后48 h、72 h数字评定量表评分均值、功能锻炼依从性差异具有统计学意义(P<0.01或P<0.05)。干预组患者术后72 h疼痛控制满意度优于对照组,差异显著(P<0.05)。结论四等级功能活动评分法用于胸腰椎术后患者进行疼痛及功能锻炼管理,可缓解术后疼痛,促进功能锻炼,改善疼痛控制满意度,提高术后早期疼痛管理质量。

Objective To explore the effect of four grade functional activity score on early pain management and functional exercise management in patients after thoracolumbar spine surgery.Methods A total of 97 patients after thoracolumbar surgery from January to June 2017 were selected as the control group,and 95 cases after thoracolumbar surgery from July to December 2017 were involved as the intervention group.The control group received the routine postoperative pain nursing care and functional exercise guidance.The intervention group established the postoperative pain intervention group,including set up patient pain nursing files,received the functional exercise guidance by four grade functional activity score and Numerical Rating Scale(NRS).Results The mean scores of NRS and adherence of functional exercises at 48 hours and 72 hours after operations were significantly different between the two groups(P<0.01 or P<0.05),the satisfaction of postoperative pain control was better in intervention than control group(P<0.05).Conclusion The application of the four grade functional activity score in patients after thoracolumbar surgery can effectively relieve the postoperative pain,improve pain control satisfaction,promote functional exercise,and improve the quality of postoperative pain management.
