摘要
目的探讨EB病毒抗体VCA-IgG、VCA-IgA、VCA-IgM和EB-DNA检测对小儿传染性单核细胞增多症(IM)的诊断作用。方法回顾性分析我院收治的78例IM患儿的临床资料,将其设为观察组,将同期在我院行健康体检的150名正常儿童作为对照组。检测两组的EB病毒抗体VCA-IgG、VCA-IgA、VCA-IgM及EB-DNA,并比较其阳性检出率。分析不同检测指标对IM诊断的灵敏度、特异度、准确度。结果观察组的EB病毒抗体VCA-IgG、VCA-IgM及EB-DNA阳性率分别为53.85%、89.74%、74.36%,均高于对照组的20.00%、2.00%、9.33%(P<0.05)。VCA-IgM联合EB-DNA对IM诊断的灵敏度、准确度高于VCA-IgG、VCA-IgA、VCA-IgM及EB-DNA单独检测(P<0.05)。结论 EB病毒抗体VCA-IgM联合EB-DNA检测应用于IM诊断中,能够提升诊断的灵敏度,减少漏诊、误诊情况。
Objective To explore the diagnostic effect of detection of EB virus antibody VCA-IgG,VCA-IgA,VCA-IgM and EB-DNA on infectious mononucleosis(IM)in children.Methods The clinical data of 78 children with IM admitted in our hospital were analyzed retrospectively,and the children were seted as observation group,150 healthy children who had routine physical examination in our hospital at the same time were seted as control group.The EB virus antibody VCA-IgG,VCA-IgA,VCA-IgM and EB-DNA of the two groups were detected,and the positive detection rates were compared.The sensitivity,specificity and accuracy of IM diagnosis with different detection indexes were analyzed.Results The positive rates of EB virus antibody VCA-IgG,VCA-IgM and EB-DNA in the observation group were 53.85%,89.74%and 74.36%,respectively,which were higher than 20.00%,2.00%,9.33%of the control group(P<0.05).The sensitivity and accuracy of VCA-IgM combined with EB-DNA in the diagnosis of IM were higher than those of VCA-IgG,VCA-IgA,VCA-IgM and EB-DNA alone(P<0.05).Conclusion The application of EB virus antibody VCA-IgM combined with EB-DNA detection in IM diagnosis can improve the diagnostic sensitivity,reduce missed diagnosis and misdiagnosis.
