环扫式超声内镜在食管癌TN分期中的诊断价值

Diagnostic value of circum-scan endoscopic ultrasonography in TN staging of esophageal carcinoma

目的分析环扫式超声内镜(EUS)在食管癌患者TN分期中的诊断价值。方法选取本院2019年7月至2020年2月本院收治的83例食管癌患者为研究对象,全部患者均行手术治疗,术前行EUS检查,根据手术病理结果分析EUS检查方法在食管癌TN分期中的诊断价值。结果 EUS诊断食管癌术前T分期的正确率为75.90%,T1、T2、T3、T4的正确率分别为57.89%、86.36%、86.21%、61.54%。EUS诊断食管癌术前N分期的正确率为63.86%,N0、N1、N2、N3的正确率分别为61.11%、60.87%、75.00%、66.67%。EUS诊断食管癌T1期的灵敏度、特异度分别为57.89%、96.88%,T2分别为86.36%、78.69%,T3分别为86.21%、92.59%,T4分别为61.54%、98.57%,N0分别为61.11%、89.23%,N1分别为60.87%、78.38%,N2分别为75.00%、82.09%,N3分别为66.67%、96.25%。结论 EUS诊断N分期的正确率较低,但诊断食管癌T2、T3分期的正确率较高,可为医师鉴别食管癌TN分期提供参考依据,具有重要的临床应用价值,值得进一步推广。

Objective To analyze the diagnostic value of circum-scan endoscopic ultrasonography(EUS)in TN staging of patients with esophageal carcinoma.Methods A total of 83 patients with esophageal carcinoma admitted in the hospital from July 2019 to December 2020 were selected as the study objects.All patients who underwent surgical treatment,and EUS examination was performed preoperatively.The diagnostic value of EUS examination in the TN staging of esophageal carcinoma was analyzed according to the surgical pathological results.Results The accuracy of EUS in the diagnosis of preoperative T staging of esophageal carcinoma was 75.90%,and that of T1,T2,T3 and T4 were 57.89%,86.36%,86.21%and 61.54%,respectively.The accuracy of EUS in the diagnosis of preoperative N staging of esophageal carcinoma was 63.86%,and that of N0,N1,N2 and N3 was 61.11%,60.87%,75.00%and 66.67%,respectively.The sensitivity and specificity of EUS in diagnosing the esophageal carcinoma were 57.89%and 96.88%for T1,86.36%and 78.69%for T2,86.21%and 92.59%for T3,61.54%and 98.57%for T4,61.11%and 89.23%for N0,60.87%and 78.38%for N1,75.00%and 82.09%for N2,and 66.67%and 96.25%for N3,respectively.Conclusion EUS has a lower accuracy in the diagnosis of N stages,but a higher accuracy in the diagnosis of T2 and T3 stages of esophageal carcinoma,it can provide a reference for physicians to differentiate the TN stages of esophageal carcinoma and has important clinical application value,which is worthy of further promotion.
