基于根本原因分析的延续性护理干预在老年脑卒中气管切开带管出院患者中的应用效果

Application effect of continuous nursing intervention based on root cause analysis in elderly stroke patients discharged with tracheotomy and tube

目的探讨基于根本原因分析(RCA)的延续性护理干预在老年脑卒中气管切开带管出院患者中的应用效果。方法选取122例气管切开带管出院的脑卒中患者作为研究对象,根据入院时间将2019年1月至6月入院的61例患者设为对照组,2019年7月至12月入院的61例患者设为观察组。对照组给予常规延续性护理干预,观察组在对照组的基础上给予基于RCA的延续性护理干预。比较两组的护理效果。结果干预后,两组的认知、行为和状态评分均升高,且观察组高于对照组(P<0.05)。观察组的管道不良事件、误吸、肺部感染发生率及再次住院率均低于对照组(P<0.05)。观察组的护理满意度优于对照组(P<0.05)。结论基于RCA的延续性护理干预可有效增强老年脑卒中气管切开带管出院患者的居家康复效果,降低并发症发生率及再次住院率,提高护理满意度。

Objective To explore the application effect of continuous nursing intervention based on root cause analysis(RCA)in elderly stroke patients discharged with tracheotomy and tube.Methods A total of 122 stroke patients discharged with tracheotomy and tube were selected as the research objects.According to the admission time,61 patients admitted from January to June 2019 were set as control group,and 61 patients from July to December 2019 were set as observation group.The control group was given routine continuous nursing intervention,and the observation group was given continuous nursing intervention based on RCA on the basis of the control group.The nursing effects of the two groups were compared.Results After intervention,the cognitive,behavioral and state scores of the two groups increased,and those in the observation group were higher than the control group(P<0.05).The incidences of pipeline adverse events,aspiration,pulmonary infection and readmission rate in the observation group were lower than those in the control group(P<0.05).The nursing satisfaction in the observation group was better than that in the control group(P<0.05).Conclusion Continuous nursing intervention based on RCA can effectively enhance the home rehabilitation effect of elderly stroke patients discharged with tracheotomy and tube,reduce the incidence of complications and readmission rate,and improve nursing satisfaction.
