摘要
目的探讨急性心衰患者肺超声积分与中性粒细胞/淋巴细胞比值(Neutrophil-to-lymphocyte ratio,NLR)关系的。方法方便选取2018年1月—2019年7月该院EICU收治的急性心衰患者62例,根据NLR分为2组:低NLR组(NLR<4.78)12例,高NLR组(NLR≥4.78)50例,并检测、比较两组肺超声28分区的积分、血肌酐、总胆红素、APACHE评分、NEWS2评分。结果低NLR组肺超声积分(37.33±13.11)、血肌酐(98.53±46.10)mg/dL、总胆红素(14.08±7.82)μmol/L、APACHE评分(32.92±8.40)分及NEWS2评分(10.33±2.87)分与高NLR组肺超声积分(47.20±19.85)、血肌酐(110.68±132.48)mg/dL、总胆红素(17.16±16.79)μmol/L、APACHE评分(32.02±7.64)分及NEWS2评分(10.18±3.18)分比较,差异无统计学意义(t=-1.633、-0.312、-0.618、0.358、0.145,P>0.05)。Spearman相关性分析结果显示急性心衰患者肺超声28分区积分与NLR无相关性(r=0.064,P>0.05)。结论肺超声积分及NLR在急性心衰患者的诊断、治疗及预后的判断中广泛应用,但两者无相关性。
Objective To investigate the lung ultrasound score and neutrophil-to-lymphocyte ratio(Neutrophil-to--lymphocyte ratio,NLR)in patients with acute heart failure.Methods A total of 62 patients with acute heart failure admitted to the EICU of the hospital from January 2018 to July 2019 were convenient selected and divided into 2 groups according to NLR:low NLR group(NLR<4.78)12 cases,high NLR group(NLR≥4.78)50 cases,blood creatinine,total bilirubin,APACHE score,and NEWS2 score of the 28-zone lung ultrasound were tested and compared between the two groups.Results Lung ultrasound score(37.33±13.11),blood creatinine(98.53±46.10)mg/dL,total bilirubin(14.08±7.82)μmol/L,APACHE score(32.92±8.40)points and NEWS2 score(10.33±2.87)points in the low NLR group compared with the high NLR group of lung ultrasound score(47.20±19.85),blood creatinine(110.68±132.48)mg/dL,total bilirubin(17.16±16.79)μmol/L,APACHE score(32.02±7.64)points and NEWS2 score(10.18±3.18)points,the difference was not statistically significant(t=-1.633,-0.312,-0.618,0.358,0.145,P>0.05).The results of Spearman correlation analysis showed that there was no correlation between the 28-zone pulmonary ultrasound score and NLR in patients with acute heart failure(r=0.064,P>0.05).Conclusion Lung ultrasound integration and NLR are widely used in the diagnosis,treatment and prognosis of patients with acute heart failure,but there is no correlation between the two groups.
