磺胺嘧啶银防治老年乳腺癌放射性皮肤损伤的效果

Effect of Silver Sulfadiazine Prevents Radioactive Skin Injury of Elderly Breast Cancer Patients

目的观察磺胺嘧啶银防治老年乳腺癌根治术患者放疗过程中放射性皮炎的临床效果。方法收集既往收治的48例乳腺癌根治术后存在放疗指征的老年患者资料,随机分为对照组和研究组,每组24例,分别在开始放疗时给予涂抹三乙醇胺乳膏和磺胺嘧啶银,评估至结束放疗后1个月。观察两组患者皮肤反应程度、疼痛评分和治疗效果之间有无差异,采用MMSA-8用药依从性问卷比较两组患者的依从性。结果两组均发生急性放射性皮肤损伤,对照组1、2级放射性皮炎分别为15例(62.5%)、9例(37.5%),观察组1、2级放射性皮炎分别为17例(70.8%)、7例(29.2%),均可减轻放射性皮炎的严重程度,无严重不良反应发生。两组在放射性皮肤损伤程度、疼痛评分之间的差异无统计学意义(P>0.05)。研究组用药依从性高于对照组,差异有统计学意义,P<0.05。结论磺胺嘧啶银能较好的防治放疗引起的急性放射性皮炎,且费用较低、患者依从性高。

Objective To investigate the clinical efficacy of Silver sulfadiazine in the treatment of radiodermatitis after radical mastectomy.Methods We collected the clinical data of 48 elderly breast cancer patients,and randomly divided into control group and experimental group.Control group(24 cases)was given trolamine cream at the beginning of radiotherapy,while experimental group(24 cases)was given Silver sulfadiazine.Observation was given until one month after the end of radiotherapy.We investigate the differences of degree of skin reaction,pain score,and treatment effect.MMSA-8 medication compliance questionnaires were used to assessment of patient′s compliance.Results The incidence of acute radioactive skin reactions was 100%,but no serious adverse reactions occurred.In the control group,there were 15 cases(62.5%)and 9 cases(37.5%)of grade 1 and 2 radioactive skin lesions,respectively.In the experimental group,there were 17 cases(70.8%)and 7 cases(29.2%)of grade 1 and 2 radioactive skin lesions,respectively.They can effectively reduce the severity of radioactive dermatitis.There was no significant difference in the degree of radiation skin injury and pain score(P>0.05).The medication compliance of the study group was higher than that of the control group,with statistical difference,P<0.05.Conclusion Silver sulfadiazine can better prevent and cure acute radioactive skin injury caused by radiotherapy.
