超声诊断妊娠晚期胎儿脐带绕颈的临床评价

Clinical Evaluation of Ultrasonic Diagnosis of Fetal Umbilical Cord Around Neck in Late Pregnancy

目的:分析超声诊断妊娠晚期胎儿脐带绕颈的价值。方法:随机选取2018年9月~2019年10月本院经超声检出的100例胎儿脐带绕颈的产妇、96例胎儿无脐带绕颈的产妇,回顾性分析所有产妇资料,将胎儿脐带绕颈的产妇设为观察组,无脐带绕颈的设为对照组,对比两组的检出率以及胎儿血管血流参数。结果:在观察组100例产妇胎儿脐带绕颈中,产后证实有95例产妇检出胎儿脐带绕颈,检出率为95.00%。观察组胎儿发生窘迫者RI和S/D检出参数明显高于未发生窘迫者和对照组正常胎儿者(P<0.05);观察组胎儿未发生窘迫者与对照组正常胎儿者RI和S/D检出参数无明显差异(P>0.05)。结论:超声检查可清晰显示胎儿脐带和脐血管血流参数,及时有效发现妊娠晚期胎儿脐带绕颈,降低胎儿宫内窘迫的发生率,保证产妇和胎儿的安全。

Objective:To discuss and analyze the value of ultrasound in diagnosing umbilical cord around neck in late pregnancy.Methods:100 cases of parturients with umbilical cord around neck and 96 cases of parturients without umbilical cord around neck detected by ultrasound in our hospital from September 2018 to October 2019 were randomly selected.All parturient data were retrospectively analyzed.The parturients with umbilical cord around neck were set as experimental group and those without umbilical cord around neck were set as control group.The detection rate and fetal vascular blood flow parameters of the two groups were compared.Results:In the experimental group of 100 parturients with umbilical cord around neck,95 parturients were confirmed to have umbilical cord around neck after delivery,with a detection rate of 95.00%.RI and S/D detection parameters in the experimental group with fetal distress were significantly higher than those in the control group without fetal distress(P<0.05).There was no significant difference in RI and S/D detection parameters between the experimental group with no fetal distress and the control group with normal fetus(P>0.05).Conclusion:Ultrasonography can clearly display the blood flow parameters of fetal umbilical cord and umbilical blood vessel,timely and effectively find the umbilical cord around the neck in the third trimester of pregnancy,reduce the incidence of fetal distress,and ensure the safety of parturient and fetus.
