摘要
贫血是慢性肾脏病(CKD)患者常见并发症之一,随着肾功能的减退,其发生率逐渐升高,程度逐渐加重。维持性血液透析(MHD)患者贫血最主要的原因是肾脏的损伤导致促红细胞生成素(EPO)的缺乏。自从1989年重组人促红细胞生成素(rHuEPO)应用于临床以来,极大地改善了MHD患者的贫血状态,进而改善了患者的长期预后和生活质量[1-3],但是仍有约5%~10%的患者对rHuEPO的反应性降低,需要更大剂量的rHuEPO治疗。然而有研究发现,过大剂量的应用rHuEPO,患者可能会出现血管通路栓塞、血压升高等不良影响,甚至增加死亡风险[4,5]。因此,对rHuEPO反应低下的患者,不应盲目增加rHuEPO的剂量,应寻找与患者EPO低反应性相关的原因。本研究旨在分析我院MHD患者rHuEPO的使用情况、反应性以及相关影响因素,为肾性贫血患者的临床个体化治疗提供重要依据。
