内皮细胞特异性分子1在早期预测肠源性脓毒症急性呼吸窘迫综合征的价值 被引量:10

Value of Endothelial Cell-Specific Molecule-1 in Early Prediction of Gut-origin Sepsis with Acute Respiratory Distress Syndrome

摘要 目的检测伴或不伴有急性呼吸窘迫综合征(ARDS)的肠源性脓毒症患者外周血内皮细胞特异性分子1(Endocan)水平,探索其在诊断肠源性脓毒症是否并发ARDS的价值。方法选取2018年1月至2019年6月南京市中心医院、南京市鼓楼医院、江苏省中西医结合医院诊治的由于各种原因所致腹腔感染造成的肠源性脓毒症患者113例,其中未合并ARDS患者55例;确诊并发ARDS患者58例。纳入时获取患者外周静脉血,采用上转发光法检测血降钙素原(PCT)及血C反应蛋白(CRP),酶联免疫吸附试验(ELISA)试剂盒检测Endocan,对比分析Endocan的价值。结果并发ARDS组患者Endocan水平低于未合并ARDS组患者(P<0.05),当患者血Endocan水平<5.36μg/L时,具有最好的诊断效率,敏感性为91%,特异性为85%。且受试者工作曲线下面积大于血CRP与PCT。结论Endocan可作为诊断肠源性脓毒症合并ARDS的检测指标。 Objective In this study,the level of endothelial cell-specific molecule-1(Endocan)in peripheral blood of gut-origin sepsis patients with or without acute respiratory distress syndrome(ARDS)was measured to explore its value in the diagnosis of gut-origin sepsis complicated with ARDS.Methods From January 2018 to June 2019,a total of 113 patients with gut-origin sepsis caused by abdominal infection of various reasons were selected from Nanjing Central Hospital,Nanjing Gulou Hospital and Jiangsu Hospital of Integrated Traditional Chinese and Western Medicine,including 55 cases without ARDS and 58 cases with confirmed ARDS.Peripheral venous blood was obtained when enrolled.Serum procalcitonin(PCT)and serum C-reactive protein(CRP)were detected by upconversion luminescence method.Endocan was detected by enzyme linked immunosorbent assay(ELISA)kit.And the value of Endocan was compared an analyzed.Results The Endocan level of patients with ARDS was lower than that of patients without ARDS(P<0.05).When the blood Endocan level was less than 5.36μg/L,the best diagnostic efficiency was achieved,with a sensitivity of 91%and specificity of 85%.And the area under the receiver operating characteristic curve was greater than that of blood CRP and PCT.Conclusion Endocan can be used as a diagnostic indicator for gut-origin sepsis complicated with ARDS.
