解毒化浊法对湿热下注型慢性肛窦炎患者免疫功能、氧化应激及炎性因子水平的影响

Effect of Detoxification and Turbidity Resolution on Immune Function,Oxidative Stress and Inflammatory Factor in Patients with Chronic Anal Sinusitis of Downward Flow of Damp-heat Syndrome

目的:观察解毒化浊法对湿热下注型慢性肛窦炎患者免疫功能、氧化应激及炎性因子水平的影响。方法:94例湿热下注型慢性肛窦炎患者按照随机数字表法随机分为中医组和对照组各47例。对照组采用左氧氟沙星片+肛肠治疗仪治疗,中医组在对照组治疗基础上联合解毒化浊法。比较两组患者免疫功能指标、氧化应激指标、炎性因子水平和中医证候评分,并比较两组患者临床疗效。结果:中医组有效率为89.36%,高于对照组的72.34%(P<0.05)。治疗后两组患者CD3^+、CD4^+、CD4^+/CD8^+明显升高(P<0.05),CD8^+明显降低(P<0.05),且中医组CD3^+、CD4^+、CD4^+/CD8^+高于对照组(P<0.05),CD8^+低于对照组(P<0.05)。治疗后,中医组IgA、IgM水平明显升高(P<0.05),IgG水平明显降低(P<0.05),对照组IgM水平明显升高(P<0.05),且中医组IgA、IgM水平高于对照组(P<0.05),IgG水平低于对照组(P<0.05)。治疗后,两组超氧化物歧化酶(superoxide dismutase,SOD)水平明显升高(P<0.05),丙二醛(malondialdehyde,MDA)、晚期氧化蛋白产物(advanced oxidation protein products,AOPPs)、8-羟基脱氧鸟苷(8-hydroxydeoxyguanosine,8-OHDG)水平明显降低(P<0.05);且中医组SOD水平高于对照组(P<0.05),MDA、AOPPs、8-OHDG水平低于对照组(P<0.05)。治疗后,两组肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)、降钙素原(procalcitonin,PCT)水平均明显降低(P<0.05),且中医组TNF-α、IL-6、CRP、PCT水平低于对照组(P<0.05)。结论:解毒化浊法治疗湿热下注型慢性肛窦炎患者临床疗效显著,能显著缓解患者症状与体征,抑制氧化应激反应,改善免疫功能紊乱和炎性因子水平。

Objective:To observe the effect of detoxification and turbidity treatment on the immune function,oxidative stress and inflammatory factor levels in patients with chronic anal sinusitis of damp-heat betting type.Methods:94 patients with chronic anal sinusitis with downward flow of Damp-heat syndrome were randomly divided into Chinese medicine group and control group with 47 cases in accordance with the random number table method.The control group was treated with levofloxacin tablets+anorectal therapy device,and the traditional Chinese medicine group was treated with detoxification and turbidity treatment based on the treatment of the control group.The immune function indexes,oxidative stress indexes,inflammatory factor levels and TCM syndrome scores were compared between the two groups of patients,and the clinical efficacy of the two groups was compared.Results:The effective rate in the TCM group was 89.36%,which was higher than 72.34%in the control group(P<0.05).After treatment,the CD3^+,CD4^+,CD4^+/CD8^+of the two groups were significantly increased(P<0.05),and CD8^+was significantly decreased(P<0.05),and the CD3^+,CD4^+,CD4^+/CD8^+of the TCM group were higher than those of the control group(P<0.05),CD8^+was lower than the control group(P<0.05).After treatment,the levels of IgA and IgM in the TCM group were significantly increased(P<0.05),and the levels of IgG were significantly reduced(P<0.05).The IgM levels in the control group were significantly increased(P<0.05),and the levels of IgA and IgM in the TCM group were higher than those of the control Group(P<0.05),the IgG level was lower than the control group(P<0.05).After treatment,the levels of superoxide dismutase(SOD)in the two groups were significantly increased(P<0.05),malondialdehyde(MDA),advanced oxidation protein products(AOPPs),8-hydroxyl Deoxyguanosine(8-hydroxydeoxyguanosine,8-OHDG)levels were significantly reduced(P<0.05);and the levels of SOD in the TCM group were higher than those in the control group(P<0.05),and the levels of MDA,AOPPs,anD8-OHDG were lower than those in the control group(P<0.05).After treatment,the two groups of tumor necrosis factor-α(tumor necrosis factor-α,TNF-α),interleukin-6(IL-6),C-reactive protein(CRP),calcium lowering The level of procalcitonin(PCT)was significantly reduced(P<0.05),and the levels of TNF-α,IL-6,CRP and PCT in the TCM group were lower than those in the control group(P<0.05).Conclusion:The method of detoxification and turbidity has a significant clinical effect in the treatment of patients with chronic anal sinusitis with downward flow of Damp-heat syndrome,which can significantly relieve the symptoms and signs of patients,inhibit oxidative stress,and improve immune dysfunction and inflammatory factor levels.
