^13C-尿素呼气试验定量值与胃窦黏膜病变程度的相关性

Correlation between ^13C-urea breath test value and severity of gastric antral mucosal lesions

目的探讨13C-尿素呼气试验定量值与胃窦黏膜病变程度的关系。方法选取2017年1月1日~2018年12月31日上海中医药大学附属龙华医院在1周内完成了13C-尿素呼气试验定量检测和胃窦黏膜组织学检查的103例住院患者的临床资料,对13C-尿素呼气试验定量值和5种胃窦黏膜病理病变评分做相关性分析。结果13C-尿素呼气试验定量值与幽门螺杆菌(Hp)数量、胃窦黏膜慢性炎症病变程度、胃窦活动性成正相关(r=0.257、0.229、0.238,P<0.05),与胃窦萎缩、肠化病变程度不相关(P>0.05)。结论临床上可根据无创的13C-尿素呼气试验定量值预测胃内的Hp数量和胃窦炎的程度,从而指导临床诊治。

Objective To explore the relationship between 13C-urea breath test(UBT)value and severity of gastric antral mucosal lesions.Methods A total of 103 cases inpatients who underwent the 13C-UBT and gastric tissue biopsy within a week in Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 1,2017 to December 31,2018 were included in the study.The severity of antral mucosal lesions was graded pathologically,and the correlation between 13C-UBT values and the five different histopathological grades was analyzed.Results The quantitative value of the 13C-UBT was positively correlated with the number of Helicobacter pylori(Hp),the degree of chronic inflammation of the gastric antral mucosa,and the activity of the gastric antrum(r=0.257,0.229,0.238,P<0.05),but was not related to the degree of antral atrophy and intestinal metaplasia(P>0.05).Conclusion Clinically,the quantitative value of the non-invasive 13C-UBT can be used to predict the number of Hp in the stomach and the degree of gastritis to guide clinical diagnosis and treatment.
