高血压患者首诊时心血管健康评分与左心室肥厚的相关性

Correlation between cardiovascular health score and left ventricular hypertrophy in patients with hypertension at first diagnosis

目的:探讨高血压患者首诊时心血管健康评分与左心室肥厚(LVH)的相关性。方法:选取2016年4月—2018年6月在汕头大学医学院第一附属医院住院的431例高血压患者为研究对象进行问卷调查,收集患者的超声心动图、体格及生化检查资料。评估高血压患者初诊时的心血管健康评分,分析其与入院时LVH的相关性。结果:高血压合并LVH患者首诊时心血管健康评分为(6.26±1.78)分,低于高血压非LVH患者(6.62±1.71分),P=0.029。多因素分析结果显示,心血管健康评分每增加1分,LVH的患病风险降低12.4%。与心血管健康评分较差组患者相比,一般组(OR=0.587,95%CI:0.359~0.961)和良好组(OR=0.508,95%CI:0.284~0.911)LVH的患病风险均降低。结论:高血压合并LVH患者初诊时的心血管健康评分较低,首诊时的心血管健康评分对评估LVH患病风险有意义。

Objective:To investigate the correlation between cardiovascular health score and left ventricular hypertrophy(LVH)in patients with hypertension at the first diagnosis.Methods:431 hypertensive patients who were hospitalized in the First Affiliated Hospital of Shantou University Medical College from April 2016 to June2018 were selected as the research subjects to conduct a questionnaire survey and collect the patients’echocardiogram,physical and biochemical examination data.To evaluate the cardiovascular health score of patients with hypertension at the first visit and analyze its correlation with LVH at admission.Results:The cardiovascular health score of patients with hypertension and LVH at the first diagnosis was(6.26±1.78)points,which was lower than that of non-LVH patients with hypertension(6.62±1.71)points,P=0.029.The results of multivariate analysis showed that for every 1 point increase in cardiovascular health score,the risk of LVH was reduced by 12.4%.Compared with the poor cardiovascular health score group,the general group(OR=0.587,95%CI:0.359-0.961)and the good group(OR=0.508,95%CI:0.284-0.911)had lower risk of LVH.Conclusion:The cardiovascular health score of patients with hypertension complicated with LVH at the first diagnosis is lower,and the cardiovascular health score at the time of first diagnosis is significant for assessing the risk of LVH.
