摘要 目的探究miR-155、miR-26a对2型糖尿病(T2DM)患者脂代谢、血管并发症的影响。方法选取本院2014年1月至2019年1月T2DM患者116例作为观察组,分为合并血管并发症亚组(81例)与未合并血管并发症亚组(35例),另选择同期健康体检者55例作为对照组。比较两组外周血miR-155、miR-26a水平,采用Logistic回归分析T2DM血管并发症的危险因素,Pearson相关性分析miR-155、miR-26a与脂代谢指标相关性,受试者工作特征(ROC)曲线评价miR-155、miR-26a预测价值。结果观察组miR-155、miR-26a高于对照组,差异有统计学意义(P<0.05);Logistic回归分析,病程、合并高血压、血清LDL-C、TC、TG、miR-155、miR-26a是T2DM患者合并血管并发症的重要危险因素(P<0.05);Pearson相关性分析,miR-155、miR-26a与LDL-C(r1=0.717、r2=0.731)、TG(r1=0.783、r2=0.745)、TC(r1=0.537、r2=0.520)呈正相关(P<0.05);ROC曲线分析,miR-155、miR-26a联合预测T2DM血管并发症的AUC为0.872,95%CI为0.797~0.927,P<0.001,诊断敏感度为72.84%,特异度为91.43%,显著优于两者单独预测。结论 miR-155、miR-26a与T2DM患者脂代谢密切相关,均为合并血管并发症的危险因素,两者联合检测能为预测血管并发症提供可靠依据。 Objective To investigate the effects of miR-155 and miR-26 a on lipid metabolism and vascular complications in patients with type 2 diabetes mellitus(T2 DM). Methods A total of 116 patients with T2 DM from January 2014 to January 2019 in our hospital were selected as the observation group,which were divided into a subgroup with vascular complications(81 cases) and a subgroup without vascular complications(35 cases). In addition,55 healthy people were selected as the control group during the same period. The peripheral blood miR-155 and miR-26 a levels were compared between the two groups. Logistic regression was used to analyze the risk factors of T2 DM vascular complications. Pearson correlation analysis was used to analyze the correlation between miR-155,miR-26 a and lipid metabolism indexes. The receiver operating characteristic(ROC)curve was used to evaluate the predictive value of miR-155 and miR-26 a. Results The miR-155 and miR-26 a in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05). Logistic regression analysis showed that the disease course,hypertension,serum LDL-C,TC,TG,miR-155,miR-26 a were important risk factors for vascular complications in T2 DM patients(P<0.05). Pearson correlation analysis showed that miR-155 and miR-26 a were positively correlated with LDL-C(r1=0.717,r2=0.731),TG(r1=0.783,r1=0.745),TC(r1=0.537,r2=0.520)(P<0.05). ROC curve analysis showed that miR-155 and miR-26 a jointly predicted AUC for T2 DM vascular complications was0.872,95% CI was 0.797~0.927,P<0.001,the diagnostic sensitivity was 72.84%,and the specificity was91.43%,which was significantly higher than the two predictions alone. Conclusion miR-155 and miR-26 a are closely related to lipid metabolism in patients with T2 DM,and are all risk factors for vascular complications.The combined detection of the two can provide a reliable basis for predicting vascular complications.
